知覺學(xué)習(xí)訓(xùn)練在弱視治療中的應(yīng)用

【摘要】: 目的:編寫“知覺學(xué)習(xí)訓(xùn)練軟件”,通過(guò)電腦顯示屏顯示空間頻率可變、刺激方式可變的知覺刺激視標(biāo),對(duì)不同類型難治性兒童弱視及成人弱視,進(jìn)行“知覺學(xué)習(xí)(perceptual learning)"訓(xùn)練。3個(gè)月后,通過(guò)訓(xùn)練前后視力、圖形視覺誘發(fā)電位(P-VEP)、明視負(fù)波(PhNR)、光學(xué)相干斷層掃描(OCT)等指標(biāo)的變化評(píng)價(jià)治療效果。 方法:1.弱視組:選擇5-35歲弱視患者48例60眼,根據(jù)視覺發(fā)育敏感期分為12歲(低齡組30眼)和≥12歲(大齡組30眼)兩個(gè)年齡組;正常對(duì)照組:5-35歲正常人30例60眼,分為12歲(低齡組30眼)和≥12歲(大齡組30眼)兩個(gè)年齡組。2.所有弱視患者均采用“位置辨別”知覺學(xué)習(xí)方法訓(xùn)練3個(gè)月。3.比較弱視組治療前后視力、P-VEP、PhNR、OCT等相應(yīng)變化。4.比較知覺學(xué)習(xí)訓(xùn)練和傳統(tǒng)弱視治療方法的有效率。5.比較弱視組與正常組P-VEP、PhNR、OCT等是否有顯著差異。 結(jié)果:1.所有弱視組與正常對(duì)照組相比P-VEP P100振幅降低、潛伏期延遲,PhNR反應(yīng)振幅降低,弱視大齡組OCT視乳頭周圍平均RNFL厚度較正常大齡組變薄,差別具有統(tǒng)計(jì)學(xué)意義。2.弱視組知覺學(xué)習(xí)訓(xùn)練前后平均視力分別為0.43、0.75,總有效率為77%,差別具有統(tǒng)計(jì)學(xué)意義。P-VEP:弱視組治療后P100振幅較治療前增大、潛伏期縮短,差別具有統(tǒng)計(jì)學(xué)意義。所有弱視組PhNR反應(yīng)振幅、潛伏期及大齡組OCT視乳頭周圍平均RNFL厚度治療前后變化無(wú)明顯統(tǒng)計(jì)學(xué)差異。3.弱視低齡組傳統(tǒng)弱視方法治療有效率經(jīng)回顧性研究為46.7%、知覺學(xué)習(xí)治療有效率為73.3%,差別具有統(tǒng)計(jì)學(xué)意義。 結(jié)論:(1)知覺學(xué)習(xí)訓(xùn)練對(duì)難治性兒童弱視及青少年、成人弱視有良好的治療效果,可以成為弱視治療的一種新方法。(2)難治性兒童弱視及青少年、成人弱視視刺激的反應(yīng)強(qiáng)度及視覺傳導(dǎo)通路、視網(wǎng)膜神經(jīng)節(jié)細(xì)胞功能均存在異常,視乳頭周圍平均RNFL厚度變薄。(3)知覺學(xué)習(xí)可以改善弱視患者視刺激的反應(yīng)強(qiáng)度及視覺傳導(dǎo)通路。(4)短期知覺學(xué)習(xí)訓(xùn)練對(duì)內(nèi)層視網(wǎng)膜節(jié)細(xì)胞功能、視乳頭RNFL形態(tài)結(jié)構(gòu)無(wú)明顯改善。
