【摘要】：目的：研究原發(fā)性閉角型青光眼患者行小梁切除術(shù)后發(fā)生淺前房的影響因素。方法：選擇2007年1月至2010年10月就診于我院行小梁切除術(shù)的原發(fā)性閉角型青光眼患者共166例201眼,其中男40例50眼,女126例151眼。記錄患者年齡、性別、青光眼類型、房角、前房深度、晶體厚度、眼軸長(zhǎng)度、瞳孔大小及光反射靈敏度、術(shù)前及術(shù)后眼壓,對(duì)術(shù)后發(fā)生淺前房患者與其余患者進(jìn)行分組數(shù)據(jù)比較(t檢驗(yàn)、卡方檢驗(yàn))及相應(yīng)的統(tǒng)計(jì)學(xué)處理。結(jié)果：在該組患者中,共發(fā)生淺前房眼58眼,發(fā)生率28.86%。淺前房組與未發(fā)生組在前方深度、眼軸長(zhǎng)度及自身兩眼解剖參數(shù)差值方面無(wú)明顯差異,晶體厚度組間有顯著性差異,房角結(jié)構(gòu)、術(shù)前眼壓、術(shù)前與術(shù)后眼壓差值、瞳孔光反射靈敏度組間均有顯著差異。術(shù)后淺前房組與未發(fā)生組在性別、年齡、青光眼類型、術(shù)后眼壓方面無(wú)明顯差異。結(jié)論：原發(fā)性閉角型青光眼患者行小梁切除術(shù)后發(fā)生淺前房與患者自身解剖結(jié)構(gòu)有關(guān),晶體較厚、術(shù)前眼壓過(guò)高、房角關(guān)閉、瞳孔光反射靈敏度差者易導(dǎo)致小梁切除術(shù)后發(fā)生淺前房。
[Abstract]:Objective: to study the influencing factors of shallow anterior chamber after trabeculectomy in patients with primary angle-closure glaucoma. Methods: a total of 166 cases (201 eyes) of primary angle-closure glaucoma were selected from January 2007 to October 2010, including 40 males (50 eyes) and 126 females (151 eyes). Age, sex, type of glaucoma, angle of atrium, depth of anterior chamber, thickness of lens, length of eye axis, pupil size, sensitivity of light reflex, intraocular pressure before and after operation were recorded. The patients with shallow anterior chamber after operation were compared with the other patients (t test, chi-square test) and the corresponding statistical processing. Results: there were 58 eyes with shallow anterior chamber in this group, the incidence was 28.86%. There was no significant difference between the shallow anterior chamber group and the non-occurring group in anterior depth, axial length and anatomic parameters of the two eyes. There were significant differences in lens thickness, atrial angle structure, intraocular pressure before and after operation, and the difference between preoperative and postoperative IOP. The sensitivity of pupillary light reflex was significantly different between the two groups. There was no significant difference in sex, age, glaucoma type and IOP between the superficial anterior chamber group and the non-occurring group. Conclusion: the shallow anterior chamber after trabeculectomy in patients with primary angle-closure glaucoma is related to the anatomical structure of the patient, the lens is thicker, the intraocular pressure is too high before operation, and the angle of atrium is closed. Poor sensitivity of pupil light reflex may lead to shallow anterior chamber after trabeculectomy.
