頻域相干光斷層掃描定量分析視網(wǎng)膜大血管腔內(nèi)直徑與原發(fā)性高血壓病相關(guān)性

【摘要】:目的探討通過(guò)頻域相干光斷層掃描(spectral domain optical coherence tomography, SD-OCT)對(duì)視網(wǎng)膜大血管“腔內(nèi)”直徑定量評(píng)價(jià)的意義,客觀評(píng)估視網(wǎng)膜大血管“腔內(nèi)”直徑的變化與原發(fā)性高血壓的相關(guān)性。 方法病例對(duì)照研究。將自2009年以來(lái),分別對(duì)為在我院就診的原發(fā)性高血壓病人群(215例)和正常血壓人群(210例)按年齡的不同分組,通過(guò)SD-OCT對(duì)眼底視網(wǎng)膜大血管“腔內(nèi)”直徑的掃描檢查,定量評(píng)估視網(wǎng)膜動(dòng)脈“腔內(nèi)”直徑(retinal artery inner caliber, RAIC)、視網(wǎng)膜靜脈的“腔內(nèi)”直徑(retinal vein inner caliber, RVIC)、以及動(dòng)靜脈“腔內(nèi)”直徑的比值(retinal arterio-venous inner caliber ratio, RAVICR),對(duì)比分析視網(wǎng)膜大血管“腔內(nèi)”直徑數(shù)據(jù)在高血壓與正常人群之間的差異;在高血壓人群內(nèi),按血壓的不同分組進(jìn)行多重比較,對(duì)比高血壓發(fā)生后,RAIC和RAVICR隨舒張壓、收縮壓變化的特點(diǎn);并對(duì)高血壓人群的RAVICR與舒張壓、收縮壓、年齡等相關(guān)因素進(jìn)行多元逐步回歸分析。 結(jié)果在各年齡組內(nèi),高血壓人群RAIC和RAVICR均較正常人群顯著下降(P0.05,t檢驗(yàn));在高血壓人群中按血壓不同分組多重比較,顯示RAIC和RAVICR均隨舒張壓、收縮壓的變化而出現(xiàn)相對(duì)的差異(P0.05,方差分析LSD檢驗(yàn));多元線逐步回歸分析顯示高血壓人群中,在a=0.05水平上,RAVICR與舒張壓和收縮壓2個(gè)因素有相關(guān)性,標(biāo)準(zhǔn)化后的回歸系數(shù)(β)分別為-0.024、-0.019,模型方差分析(ANOVA)中的F=11.231,P=0.001,有統(tǒng)計(jì)學(xué)意義。 結(jié)論借助SD-OCT可對(duì)視網(wǎng)膜大血管“腔內(nèi)”直徑進(jìn)行對(duì)比分析;RAIC和RAVICR的變化與原發(fā)性高血壓的舒張壓和收縮壓變化有相關(guān)性。
[Abstract]:Objective to investigate the significance of frequency domain coherence tomography (spectral domain optical coherence tomography, SD-Oct) in the quantitative evaluation of the "intracavity" diameter of retinal macrovascular, and to evaluate objectively the correlation between the change of "intracavity" diameter of retinal macrovascular and essential hypertension. Methods A case-control study was conducted. Since 2009, a group of 215 patients with essential hypertension (EH) and 210 patients with normal blood pressure (NBP) were divided into two groups according to age. The diameter of retinal vessels in the fundus was examined by SD-Oct. The retinal artery "intracavity" diameter (retinal artery inner caliber,), the retinal vein "intracavity" diameter (retinal vein inner caliber, Ric), and the ratio of arteriovenous "intracavity" diameter (retinal arterio-venous inner caliber ratio, RAVICR) were quantitatively evaluated. "differences in diameter data between hypertensive and normal populations; In the hypertensive population, the changes of RAIC and RAVICR with diastolic blood pressure and systolic blood pressure were compared according to different groups of blood pressure, and the changes of RAVICR and diastolic blood pressure and systolic blood pressure were compared. Multiple stepwise regression analysis was carried out for age and other related factors. Results in all age groups, the Raic and RAVICR in hypertensive population were significantly lower than those in normal subjects (P0.05t test), and the results showed that Raic and RAVICR were both with diastolic blood pressure (DBP). There were relative differences in systolic blood pressure (P0.05, ANOVA LSD test), multiple linear stepwise regression analysis showed that RAVICR was correlated with diastolic blood pressure and systolic blood pressure at a0.05 level. The standardized regression coefficients (尾) were -0.024 ~ 0.019, respectively, and the model variance analysis (ANOVA) showed a significant difference in the analysis of variance (ANOVA). Conclusion SD-Oct can be used to compare the "intracavity" diameters of large retinal vessels and to analyze the correlation between the changes of RAIC and RAVICR and the changes of diastolic and systolic blood pressure in essential hypertension.
