【摘要】： 目的 通過主客觀評估方法分析咽喉反流(laryngopharyngeal reflux, LPR)引起嗓音障礙疾病的嗓音功能特點(diǎn)及對患者生活質(zhì)量的影響,分析客觀評估與主觀評估的相關(guān)性。 方法 對112例發(fā)音障礙可疑有咽喉反流的患者進(jìn)行電子鼻咽喉鏡檢查及反流癥狀指數(shù)量表(the reflux symptom index, RSI)和反流檢查計(jì)分量表(The reflux finding score, RES)評估,將其中RSI評分13分和(或)RES評分7分定為陽性。兩個量表均為陽性診斷為咽喉反流的患者共有50例,對這50例患者進(jìn)行嗓音障礙指數(shù)量表(voice handicap index, VHI)評估及嗓音聲學(xué)分析及電聲門圖檢查,部分行胃食道鏡檢查。 結(jié)果 診斷為咽喉反流的50例患者完成問卷調(diào)查、VHI評估及嗓音聲學(xué)分析及電聲門圖檢查,部分接受胃食道鏡檢查。咽喉反流發(fā)音障礙患者的發(fā)病與年齡、性別、生活習(xí)慣等密切相關(guān)。與正常對照組36例志愿者比較,基頻、基頻微擾、接觸率及最大發(fā)聲時間差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。VHI量表評估生理P評分最高,其次為功能F評分,情感E評分最低。咽喉反流患者的RFS與VHI有一定相關(guān)性(P0.05),而RSI與RFS、VHI無明顯相關(guān)性(P0.05)。 結(jié)論 咽喉反流造成的聲帶損傷已經(jīng)影響到發(fā)聲功能,主要表現(xiàn)在聲帶振動不穩(wěn)定,聲門閉合不良,接觸時間延長,發(fā)聲能力下降,對患者的情感方面的影響往往不受患者重視。評估嗓音的客觀差異和嗓音對患者生活質(zhì)量的影響,為咽喉反流疾病的診斷、治療和預(yù)防提供科學(xué)依據(jù)。在判斷治療效果,隨診動態(tài)觀察等具有廣泛意義,能對患者的依從性產(chǎn)生積極的影響并提高醫(yī)師對咽喉反流的患者長期治療的信心。
[Abstract]:Objective to analyze the characteristics of voice function and its influence on the quality of life of patients with voice disorders caused by throat reflux (laryngopharyngeal reflux,) by subjective and objective evaluation, and to analyze the correlation between objective assessment and subjective assessment. Methods one hundred and twelve patients with suspected pharyngeal reflux were evaluated by electronic laryngoscopy and index of reflux symptoms (the reflux symptom index, RSI) and the reflux finding score, res. The RSI score was 13 and / or res score was 7. There were 50 patients with pharynx and larynx reflux diagnosed by the two scales. The voice disturbance index scale (voice handicap index, VHI), voice acoustic analysis and electroglottic examination were performed in 50 patients, and some gastroesophageal examinations were performed. Results 50 patients with pharynx and larynx reflux completed VHI evaluation voice acoustic analysis and electroglottography partly underwent gastroesophageal examination. The incidence of throat reflux dysphonia is closely related to age, sex, life habits and so on. Compared with 36 normal volunteers, the differences of basal frequency perturbation, contact rate and maximal vocal time were statistically significant (P0.01) .VHI scale evaluated physiological P score was the highest, followed by functional F score, emotion E score was the lowest. There was a certain correlation between RFS and VHI in patients with throat reflux (P0.05), but there was no significant correlation between RSI and RFS- VHI (P0.05). Conclusion vocal cord injury caused by pharynx and larynx reflux has affected vocal function, mainly in unstable vocal cord vibration, poor glottis closure, prolonged contact time and decreased vocal ability. Emotional impact on patients is often ignored. Objective differences of voice and influence of voice on patients' quality of life were evaluated to provide scientific basis for diagnosis, treatment and prevention of throat reflux disease. It is of great significance to judge the therapeutic effect and follow up dynamic observation, which can have a positive effect on the compliance of the patients and enhance the doctors' confidence in the long-term treatment of the patients with pharynx and throat reflux.
