【摘要】： 背景和目的 本文通過研究比較一眼植入AcrySof ReSTOR (SA60D3)多焦點人工晶狀體,對側(cè)眼植入AcrySof Natural (SA60AT)單焦點人工晶狀體及雙眼均植入AcrySof Natural (SA60AT)單焦點人工晶狀體術(shù)后人工晶體眼的視功能的觀察分析,評價單眼植入AcrySof ReSTOR多焦點人工晶狀體的臨床應(yīng)用價值。 資料和方法 觀察2008年10月～2009年11月在我院接受白內(nèi)障超聲乳化吸除聯(lián)合人工晶狀體植入的40例(80眼)患者按有否脫鏡需求分組,實驗組：雙眼分別植入AcrySof ReSTOR IOL和AcrySof Natural IOL 20例(40眼),對照組：雙眼植入AcrySof Natural IOL 20例(40眼)；均為非同期手術(shù),雙眼手術(shù)后隨訪3～5月,觀察手術(shù)后患者的裸眼遠近視力、最佳矯正遠近視力、立體視覺、對比敏感度及患者的滿意度,以問卷的形式對患者術(shù)后的主觀感覺進行調(diào)查。 結(jié)果 術(shù)后1周及術(shù)后3月的UCDNV、BCDNV、BCNVA實驗組與對照組之間比較差異無統(tǒng)計學(xué)意義(χ2=2.975,1.554,1.779,2.256,1.954,1.441；P0.05),實驗組在術(shù)后1周及術(shù)后3月的UCNVA與對照組的差異有統(tǒng)計學(xué)意義(χ2=15.667,16.035；P0.01)；術(shù)后3月實驗組與對照組的同時視功能和融合功能之間比較差異無統(tǒng)計學(xué)意義(χ2=0.974,0.974；P0.05)；術(shù)后3月UCDNV下的遠立體視銳度實驗組與對照組比較差異無統(tǒng)計學(xué)意義(χ2=2.222；P0.05)；術(shù)后3月UCNVA、BCNVA下的近立體視銳度實驗組與對照組比較差異無統(tǒng)計學(xué)意義(χ2=2.465,2.378；P0.05)；術(shù)后1月兩組的對比敏感度在各空間頻率上差異有統(tǒng)計學(xué)意義(P0.05)；術(shù)后3月兩組的對比敏感度在各空間頻率上差異無統(tǒng)計學(xué)意義(P0.05)。調(diào)查問卷顯示,單眼植入AcrySof ReSTOR多焦點人工晶狀體術(shù)后患者滿意度高。 結(jié)論 與雙眼均植入單焦點人工晶狀體相比,單眼植入AcrySof ReSTOR多焦點人工晶狀體組患者在沒有損失遠視力、遠立體視力、沒有明顯降低對比敏感度、同時視功能和融合功能的前提下,獲得了良好的近視力,且近、中距離視功能滿意度高,患者脫鏡率較高,減少了對眼鏡的依賴程度,具有一定的臨床應(yīng)用價值。
[Abstract]:Background and objective to compare the effects of implantation of AcrySof rest (SA60D3) intraocular lens. To evaluate the clinical value of unilateral implantation of AcrySof Natural (SA60AT) single focus intraocular lens and binocular implantation of AcrySof Natural (SA60AT) single focus intraocular lens (IOL). Materials and methods from October 2008 to November 2009, 40 patients (80 eyes) who received phacoemulsification combined with intraocular lens implantation in our hospital were divided into groups according to whether or not the lens was removed. In the experimental group, 20 cases (40 eyes) were implanted with AcrySof natural IOL (40 eyes), and 20 cases (40 eyes) were implanted with AcrySof Natural IOL (40 eyes). The best corrected far and near visual acuity, stereopsis, contrast sensitivity and patient satisfaction were investigated by questionnaire. Results there was no significant difference between the experimental group and the control group at 1 week and 3 months after operation (蠂 ~ 2 / 2.975 ~ 1.554 ~ 1.779 ~ 2.256 ~ 1.9541.441P 0.05). The UCNVA of the experimental group was significantly different from that of the control group at 1 week after operation and 3 months after operation (蠂 ~ 2 ~ (2) ~ (15) 667U ~ (16.035) P ~ (0.01), and there was no significant difference between the experimental group and the control group (蠂 ~ (2) at 1 week and 3 months after operation (蠂 ~ (2) 2.975). There was no significant difference in the simultaneous visual function and fusion function between the experimental group and the control group 3 months after operation (蠂 ~ 2 / 0. 974 ~ 0. 974), but there was no significant difference in the acuity of far stereopsis between the experimental group and the control group 3 months after operation (蠂 2 / 2. 222). There was no significant difference in the near stereopsis acuity between the experimental group and the control group 3 months after operation (蠂 2 / 2.465 / 2.378g P 0.05), but the contrast sensitivity of the two groups was significantly different in all spatial frequencies at 1 month after operation (P0.05). There was no significant difference in contrast sensitivity between the two groups in each spatial frequency 3 months after operation (P0.05). The questionnaire showed that patients with monocular implantation of AcrySof ReSTOR had high satisfaction after multi-focus intraocular lens implantation. Conclusion compared with the single focus intraocular lens implantation in both eyes, the patients with single eyes implanted with AcrySof ReSTOR multifocal intraocular lens have no loss of far vision, distant stereoscopic vision, and no significant decrease in contrast sensitivity. On the premise of both visual function and fusion function, good near vision was obtained, and close and middle distance visual function satisfaction was high, and the rate of devitalization of patients was higher, which reduced the degree of dependence on glasses and had certain clinical application value.
