【摘要】：目的:比較小劑量克拉霉素與鼻內(nèi)鏡術(shù)治療慢性鼻竇炎的臨床療效和安全性。方法:將符合標(biāo)準(zhǔn)的98例慢性鼻竇炎患者隨機(jī)分為藥物組和手術(shù)組。藥物組患者給予克拉霉素分散片250 mg,口服,bid,1周后改為qd。手術(shù)組患者采用鼻內(nèi)鏡手術(shù)治療。兩組患者均輔助鼻腔沖洗和局部外用類固醇激素,療程均為3個(gè)月。觀察兩組患者的臨床療效,治療前后視覺(jué)模擬量表(VAS)評(píng)分、鼻內(nèi)鏡檢查(Lund-Mackay)評(píng)分,平均治療費(fèi)用,并發(fā)癥及不良反應(yīng)發(fā)生情況。結(jié)果:兩組患者總有效率比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。治療后,兩組患者VAS評(píng)分、Lund-Mackay評(píng)分均顯著低于同組治療前,差異均有統(tǒng)計(jì)學(xué)意義(P0.05),但兩組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。藥物組患者平均治療費(fèi)用、并發(fā)癥發(fā)生率、不良反應(yīng)發(fā)生率均顯著低于手術(shù)組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:小劑量克拉霉素與鼻內(nèi)鏡術(shù)治療慢性鼻竇炎的療效相當(dāng),但克拉霉素方案在治療費(fèi)用、安全性方面優(yōu)于鼻內(nèi)鏡術(shù)。
[Abstract]:Objective: to compare the efficacy and safety of low dose clarithromycin and endoscopic sinus surgery in the treatment of chronic sinusitis. Methods: 98 patients with chronic sinusitis were randomly divided into drug group and operation group. Patients in the drug group were given clarithromycin dispersible tablets 250 mg. The patients in the operation group were treated with endoscopic sinus surgery. The patients in both groups were treated with nasal irrigation and topical steroid hormone for 3 months. The clinical efficacy, visual analogue scale (VAS) score, Lund-Mackay score, average treatment cost, complications and adverse reactions were observed. Results: there was no significant difference in total effective rate between the two groups (P0.05). After treatment, the VAS scores and Lund-Mackay scores of the two groups were significantly lower than those of the same group before treatment, the differences were statistically significant (P0.05), but there was no significant difference between the two groups (P0.05). The average cost of treatment, incidence of complications and adverse reactions in the drug group were significantly lower than those in the operation group (P0.05). Conclusion: the efficacy of low dose clarithromycin in treating chronic sinusitis is similar to that of endoscopic sinus surgery, but clarithromycin regimen is superior to endoscopic sinus surgery in cost and safety.
