【摘要】：目的：探討Twist在鼻咽癌中的表達情況及臨床意義。 研究方法：采用免疫組化法檢測83例鼻咽癌和18例鼻咽部炎性組織中Twist的表達。對鼻咽癌組織中Twist的表達與臨床病理因素及預后的關系進行統(tǒng)計學分析。 結果：1、Twist在鼻咽部炎性組織中不表達,在鼻咽癌組織中的高表達率為49.4%,兩者有顯著的統(tǒng)計學差異(P=0.000)。 2、Twist表達與患者的年齡、性別及T分期無明顯相關性(PO.05)。但是Twist高表達患者較低表達患者具有更高的淋巴結轉移率(P0.001)、遠處轉移率(P=0.023)以及復發(fā)率P=0.040)。Twist的表達與淋巴結轉移(r=0.414,P0.001)、遠處轉移(r=0.250,P=0.023)、復發(fā)存在正相關(r=0.226,P=0.04)。 3、Twist高表達組5年生存率為46.34%,Twist低表達組5年生存率為Twist74.42%,兩組比較有顯著統(tǒng)計學意義(P=0.007)。單因素分析顯示鼻咽癌患者5年生存率與T分期、淋巴結轉移情況、遠處轉移、TNM分期、Twist的表達水平相關(P0.05)。多因素分析顯示淋巴結轉移情況、遠處轉移、TNM分期及Twist的表達水平為鼻咽癌患者5年生存率的獨立影響因素(P0.05)。 結論：Twist在鼻咽部炎性組織中不表達,在鼻咽癌組織中有較高的陽性表達率。且與鼻咽癌的淋巴結轉移、遠處轉移、以及復發(fā)密切相關。Twist可能作為判斷鼻咽癌轉移潛能和預后的輔助指標。
[Abstract]:Objective: to investigate the expression and clinical significance of Twist in nasopharyngeal carcinoma. Methods: the expression of Twist in 83 cases of nasopharyngeal carcinoma and 18 cases of nasopharyngeal inflammatory tissue was detected by immunohistochemical method. The relationship between Twist expression and clinicopathological factors and prognosis in nasopharyngeal carcinoma was analyzed statistically. Results there was no expression of Twist in nasopharyngeal inflammatory tissues, but a high expression rate of 49.4% in nasopharyngeal carcinoma (P < 0.000), but there was no significant correlation between the expression of Twist and age, sex and T stage (PO.05). But the patients with high expression of Twist had higher lymph node metastasis rate (P0.001), distant metastasis rate (P0. 023) and recurrence rate (P0. 040). The 5-year survival rate in Twist low expression group was Twist74.42, which was statistically significant (P0. 007). Univariate analysis showed that the 5-year survival rate was correlated with T stage, lymph node metastasis and the expression of Twist in TNM staging (P0.05). Multivariate analysis showed that lymph node metastasis, distant metastasis TNM stage and Twist expression level were independent factors of 5-year survival rate in nasopharyngeal carcinoma (P0.05). Conclusion Twist is not expressed in nasopharyngeal inflammatory tissue, but has a high positive rate in nasopharyngeal carcinoma. It was suggested that. Twist was closely related to lymph node metastasis, distant metastasis and recurrence of nasopharyngeal carcinoma (NPC), and might be an auxiliary index to judge metastasis potential and prognosis of nasopharyngeal carcinoma (NPC).
