【摘要】：目的： 應(yīng)用光學(xué)相干斷層掃描儀(Optical Coherence Tomography OCT)觀察糖尿病性白內(nèi)障患者接受白內(nèi)障超聲乳化聯(lián)合人工晶體植入術(shù)后黃斑中心凹視網(wǎng)膜厚度(Foveal retinal thickness, FRT)變化規(guī)律和其與視力的相關(guān)性,以及術(shù)后加用普拉洛芬眼液對(duì)其的影響。 方法： 選擇2010年2月至2011年1月單純年齡相關(guān)性白內(nèi)障患者(62眼)及糖尿病性白內(nèi)障患者(60眼),均于中南大學(xué)湘雅二醫(yī)院眼科接受白內(nèi)障超聲乳化聯(lián)合折疊式人工晶體植入術(shù),及醫(yī)院體檢的正常人(30眼)共152眼。分為4組：A組：?jiǎn)渭兡挲g相關(guān)性白內(nèi)障組31眼；B組：糖尿病性白內(nèi)障組32眼,其中B1組糖尿病病程≤10年10眼,B2組糖尿病病程10年22眼；C組：術(shù)后加用普拉洛芬眼液組59眼,其中C1組無(wú)糖尿病31眼,C2組糖尿病病程≤10年11眼,C3組糖尿病病程10年17眼；D組：正常組30眼。OCT檢測(cè)A、B、C各組術(shù)前及術(shù)后1周、術(shù)后1月、術(shù)后3月黃斑中心凹視網(wǎng)膜厚度及D組的黃斑中心凹視網(wǎng)膜厚度,比較各組及各組間手術(shù)前后黃斑中心凹視網(wǎng)膜厚度的差別,分析術(shù)后視力與黃斑中心凹視網(wǎng)膜厚度的關(guān)系及普拉洛芬眼液對(duì)黃斑水腫的防治作用。 結(jié)果： 1.各組術(shù)前FRT分別為：A組152.00±10.14um,B1組149.10±15.96um,B2組155.95±17.18um,C1組158.27±16.33um,C2組160.09±19.62um,C3組157.47±18.35um,D組158.55±12.04um,各組比較均無(wú)顯著性差異(P0.05)。 2.A組術(shù)前與術(shù)后1周,1月,3月的FRT分別為：152.00±10.14um,165.2±23.93um、165.5±16.37um、157.2±20.48um。與術(shù)前相比,術(shù)后各時(shí)間點(diǎn)FRT無(wú)顯著性差異(P0.05)。 3.B組術(shù)前與術(shù)后1周,1月,3月的FRT分別為：B1組：149.10±15.96um、193.8±31.73um、184.7±22.37um、158.5±15.13um; B2組：155.95±17.18um、212.4±29.09um、206.6±28.56um、171.4±16.37um.與術(shù)前相比,B1組在術(shù)后1周及1月FRT增厚,差異有統(tǒng)計(jì)學(xué)意義(P0.05)；在術(shù)后3月厚度無(wú)顯著性差異(P0.05)。B2組術(shù)后1周、1月及3月時(shí)FRT增厚,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。 4.C組術(shù)前與術(shù)后1周,1月,3月的FRT分別為：C1組：158.27±16.33um、169.1±22.83 um、170.1±20.02 um、158.8±19.36 um; C2組：160.09±19.62um、182.6±22.88 um、182.6±15.81um、165.9±19.85um; C3組：157.47±18.35um、186.6±35.78 um、182.9±26.63um、164.2±21.36um.與術(shù)前相比,C1組術(shù)后各時(shí)間點(diǎn)FRT無(wú)顯著性差異(P0.05)。C2組及C3組術(shù)后1周及1月時(shí)FRT增厚,差異有統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)后3月時(shí)FRT無(wú)顯著性差異(P0.05)。 5.A組與B1組、B2組、C1組術(shù)后比較FRT增厚有統(tǒng)計(jì)學(xué)意義的是：B1組術(shù)后1周及術(shù)后1月；B2組術(shù)后1周,術(shù)后1月及術(shù)后3月(P0.05)；余FRT無(wú)顯著性差異(P0.05) 6.B1組與B2組術(shù)后比較：B2組術(shù)后1月FRT較B1組明顯增厚,兩組間有顯著性差異(P0.05)；術(shù)后1周及術(shù)后3月兩組間無(wú)顯著性差異(P0.05)。 7.B1組與C2組術(shù)后比較：B1組術(shù)后各觀測(cè)時(shí)間點(diǎn)FRT與C2組無(wú)顯著性差異(P0.05)。 8.B2組與C3組術(shù)后比較：B2組術(shù)后1周及術(shù)后1月FRT較C3組明顯增厚,兩組間有顯著性差異(P0.05)。術(shù)后3月兩組間無(wú)顯著性差異(P0.05)。 9.各組術(shù)眼FRT與視力的相關(guān)性：呈負(fù)相關(guān)的有：B1組術(shù)后1月時(shí)(r=－0.764,P0.05)；B2組術(shù)后1月及3月時(shí)(r=-0.892,-0.164；P0.05)；C2組術(shù)后1月時(shí)(r=－0.870；P0.05)；C3組術(shù)后1月時(shí)(r=-0.758；P0.05)。無(wú)相關(guān)性的有：A組術(shù)后1月,3月時(shí)(r＝0.109,－0.096；P0.05)；B1組術(shù)后3月時(shí)(r=-0.055；p0.05)；C1組術(shù)后1月,3月時(shí)(r=－0.253,－0.364；P0.05)；C2組術(shù)后3月時(shí)(r=-0.544；p0.05)；C3組術(shù)后3月時(shí)(r=-0.469；p0.05)。 結(jié)論 1.糖尿病性白內(nèi)障患者接受超聲乳化聯(lián)合人工晶體植入術(shù)后其黃斑中心凹視網(wǎng)膜較單純老年性白內(nèi)障患者顯著增厚,糖尿病病程10年以內(nèi)者多在術(shù)后3月內(nèi)恢復(fù)。 2.普拉洛芬眼液可減輕白內(nèi)障超聲乳化聯(lián)合人工晶體植入術(shù)后術(shù)眼前節(jié)炎癥反應(yīng),對(duì)于糖尿病病程10年以上患者還可減輕術(shù)后黃斑中心凹網(wǎng)膜增厚程度。 3.在術(shù)后黃斑中心凹視網(wǎng)膜厚度異常增厚的患者中,視力與中心凹網(wǎng)膜厚度呈負(fù)相關(guān)。 4.OCT是研究白內(nèi)障患者術(shù)后黃斑變化的一種理想工具。
[Abstract]:Objective:
Optical Coherence Tomography OCT was used to observe the changes of the macular fovea retinal thickness (Foveal retinal thickness, FRT) and its correlation with visual acuity after cataract phacoemulsification and intraocular lens implantation in diabetic cataract patients after cataract phacoemulsification and intraocular lens implantation, and the Pla Lo Finn eye solution was added after operation. Method:
From February 2010 to January 2011, patients with simple age related cataract (62 eyes) and diabetic cataract (60 eyes) were treated with cataract phacoemulsification and foldable intraocular lens implantation in the Department of Ophthalmology, Xiangya No.2 Hospital of Central South University, and 152 eyes of normal persons (30 eyes) in hospital physical examination.
