【摘要】： [目的]比較虹膜定位引導(dǎo)的個(gè)性化準(zhǔn)分子激光原位角膜磨鑲術(shù)(IR-LASIK)與傳統(tǒng)準(zhǔn)分子激光原位角膜磨鑲術(shù)(LASIK)治療中高度散光的臨床效果。 [方法]將符合入選條件的2.0DC以上的近視性散光患眼隨機(jī)分組,分別使用Carl Zeiss Mel80準(zhǔn)分子激光機(jī)進(jìn)行IR-LASIK和LASIK手術(shù),隨訪3個(gè)月,觀察術(shù)后裸眼視力、最佳矯正視力、球鏡度、殘留散光度、高階像差及對(duì)比敏感度,對(duì)兩組在安全性、有效性、穩(wěn)定性及可預(yù)測(cè)性等方面進(jìn)行比較；結(jié)果應(yīng)用SPSS 16.0統(tǒng)計(jì)軟件進(jìn)行分析處理,P0.05為有統(tǒng)計(jì)學(xué)意義。 [結(jié)果]對(duì)50例63眼進(jìn)行統(tǒng)計(jì)分析。術(shù)后三個(gè)月：IR-LASIK組安全指數(shù)(SI)1.14,高于LASIK組(1.07),兩者比較無(wú)顯著性差異(P=0.321)；裸眼視力IR-LASIK組54眼(93.55%)≥1.0,LASIK組25眼(78.13%)≥1.0,兩者比較有顯著性差異(P=0.049)；80.96%眼殘留散光在±0.5D以內(nèi)(其中IR-LASIK組30眼、LASIK組23眼),兩者比較有顯著性差異(P=0.009)；IR-LASIK組2周與1月、1月與3月的平均等效球鏡差值分別為0.12D、0.18D,LASIK組則均為0.19D,兩組比較無(wú)顯著性差異(P=0.373)；兩組獲得的等效球鏡矯正度數(shù)與術(shù)前的等效球鏡擬矯正度數(shù)呈高度線性相關(guān),兩者比較無(wú)顯著性差異(P=0.241)；高階像差(HOA)、彗差、三葉草和球差的均方根值IR-LASIK組分別為0.47μm,0.74μm,0.48μm,0.53μm,LASIK組則為0.54μm,0.93μm,0.48μm,0.73μm,兩組比較無(wú)顯著性差異(P0.05)；各空間頻率對(duì)應(yīng)的眩光對(duì)比敏感度均值明顯超過(guò)術(shù)前,高空間頻率(6.6,10.4 cpd)對(duì)應(yīng)的對(duì)比敏感度均值恢復(fù)至術(shù)前水平,低空間頻率(1.0,1.7,2.6,4.2 cpd)的對(duì)比敏感度均值仍未恢復(fù)至術(shù)前水平,兩組比較無(wú)顯著性差異(P0.05)�！窘Y(jié)論】虹膜定位引導(dǎo)的個(gè)性化準(zhǔn)分子激光原位角膜磨鑲術(shù)治療中高度散光安全有效,與傳統(tǒng)的準(zhǔn)分子激光原位角膜磨鑲術(shù)相比術(shù)后殘留散光更少,裸眼視力更佳,可以獲得更好的術(shù)后視覺(jué)質(zhì)量。
[Abstract]:[objective] to compare the clinical effect of Iris guided individualized laser in situ keratomileusis (IR-LASIKI) and traditional laser in situ keratomileusis (LASIK) in the treatment of high astigmatism. [methods] patients with myopic astigmatism above 2.0DC were randomly divided into two groups. IR-LASIK and LASIK were operated on by Carl Zeiss Mel80 excimer laser machine, followed up for 3 months to observe the uncorrected visual acuity, the best corrected visual acuity and the degree of spherical endoscopy. Residual astigmatism, high-order aberration and contrast sensitivity were compared between the two groups in terms of safety, effectiveness, stability and predictability. [results] A statistical analysis was made on 63 eyes of 50 cases. Three months after operation, the safety index of the IR-LASIK group was 1.14, which was higher than that of the LASIK group. There was no significant difference between the two groups. There was no significant difference in the residual astigmatism within 鹵0.5D (in the IR-LASIK group, the residual astigmatism was within 鹵0.5D in the IR-LASIK group (54 eyes, 93.555.55%) 鈮，
