外耳道及中耳膽脂瘤發(fā)生發(fā)展過程的動物實驗研究

【摘要】: 目的 應(yīng)用蒙古長爪沙土鼠建立外耳道及中耳膽脂瘤模型并動態(tài)觀察膽脂瘤的局部生長及破壞方式。為進一步研究膽脂瘤的發(fā)病機制并進行相關(guān)藥物實驗研究奠定基礎(chǔ)。 方法 選取月齡2-6個月的雄性長爪蒙古沙土鼠50只,分為五組,每組10只動物雙側(cè)外耳道及鼓膜鏡檢均正常,全部于麻醉后行改良的右外耳道結(jié)扎術(shù),左耳為對照耳。分別于術(shù)后15、30、45、60、90天解剖外耳道及雙側(cè)聽泡,觀察膽脂瘤在沙鼠外耳道及聽泡內(nèi)的生長及對周圍結(jié)構(gòu)的破壞情況并進行分期,再將膽脂瘤取出做組織病理學檢查。 結(jié)果 50只沙土鼠共成模38只,總成模率為76%,其中15天時成模率為50%,30天時成模率為60%,45天時成模率為80%,60天時成模率為90%,90天時成模率為100%;開放鼠雙側(cè)聽泡,可見膽脂瘤生長進入右側(cè)聽泡內(nèi),并且隨著時間延長侵犯范圍逐漸擴大;組織病理檢查見取出的標本有明顯囊袋形成,囊袋外層由炎細胞和纖維組織包裹,內(nèi)面為復層鱗狀上皮,囊內(nèi)含有大量脫落上皮及角化物,同時可見大量炎細胞浸潤、粘液滲出及鈣化灶,確定為膽脂瘤改變。 結(jié)論 應(yīng)用外耳道結(jié)扎法可以很簡便易行地建立外耳道及中耳膽脂瘤的動物模型,成模率較高;模型鼠膽脂瘤及其發(fā)生發(fā)展過程與人類膽脂瘤具有基本相同的特性;利用蒙古長爪沙鼠建立的外耳道及中耳膽脂瘤動物模型是研究膽脂瘤的發(fā)生發(fā)展及其破壞行為的理想試驗材料,同時為進一步進行膽脂瘤相關(guān)藥物實驗研究奠定了良好的動物實驗基礎(chǔ)。
[Abstract]:Objective to establish the model of cholesteatoma in external auditory canal and middle ear of gerbils mongolicus and observe the local growth and destruction of cholesteatoma dynamically. Methods 50 male Mongolian gerbils aged 2-6 months were selected and divided into five groups. The bilateral external auditory meatus and tympanic membrane were all normal in each group of 10 animals, all of them were treated with modified right external auditory canal ligation after anesthesia, and the left ear was used as control ear. The external auditory meatus and bilateral auditory vesicles were dissected at 15: 30 and 45, 6090 days after operation. The growth of cholesteatoma in the external auditory canal and auditory vesicle of gerbils and the destruction of surrounding structures were observed and divided into stages. The cholesteatoma was removed for histopathological examination. The assembly mold rate is 76. In 15 days, the model forming rate is 50 and 30 days, and the model forming rate is 60 and 45 days. The model forming rate is 80 or 60 days and the model forming rate at 90 days is 100. When the bilateral auditory bubbles are opened, it can be seen that cholesteatoma grows into the right auditory vesicles. Histopathological examination showed that the specimens had obvious sac formation, the outer layer of the capsule was covered with inflammatory cells and fibrous tissue, and the inner surface was laminated squamous epithelium. The capsule contains a large number of exfoliative epithelium and keratocytes, as well as a large number of inflammatory cell infiltration, mucus exudation and calcification. Conclusion the animal model of external auditory canal and middle ear cholesteatoma can be easily established by ligation of external auditory canal and middle ear cholesteatoma. The model rat cholesteatoma and its occurrence and development process have the same characteristics as human cholesteatoma. The animal model of external auditory canal and middle ear cholesteatoma established by Mongolian gerbils is an ideal experimental material for studying the occurrence, development and destructive behavior of cholesteatoma. At the same time, it lays a good animal experimental foundation for the further study of cholesteatoma related drugs.
