【摘要】：目的比較分析內(nèi)眼手術(shù)患者術(shù)前使用抗菌藥物滴眼聯(lián)合結(jié)膜囊沖洗及全身使用抗菌藥物的結(jié)膜囊無菌化效果。方法隨機(jī)選擇2012年4月-2014年2月收治的內(nèi)眼手術(shù)100眼,分為對(duì)照組25眼及試驗(yàn)組75眼,試驗(yàn)組隨機(jī)分為3組,試驗(yàn)組和對(duì)照組患者均在未干預(yù)前實(shí)施結(jié)膜囊細(xì)菌培養(yǎng);對(duì)照組患者均采用生理鹽水進(jìn)行沖洗,試驗(yàn)1組采用左氧氟沙星滴眼、試驗(yàn)2組采用左氧氟沙星滴眼聯(lián)合生理鹽水沖洗、試驗(yàn)3組采用左氧氟沙星滴眼、生理鹽水沖洗聯(lián)合全身抗菌藥物靜脈滴注;兩組患者均于用藥后第3天對(duì)結(jié)膜囊采樣進(jìn)行細(xì)菌培養(yǎng),采用SPSS15.0統(tǒng)計(jì)軟件進(jìn)行分析。結(jié)果對(duì)照組及3個(gè)試驗(yàn)組采用感染預(yù)防措施前檢出率均為36.0%,實(shí)施感染預(yù)防措施后,病原菌檢出率對(duì)照組為24.0%、試驗(yàn)1組為12.0%、試驗(yàn)2組為8.0%、試驗(yàn)3組為4.0%,組間差異有統(tǒng)計(jì)學(xué)意義(P0.05);干預(yù)后結(jié)膜囊細(xì)菌培養(yǎng)陽性呈現(xiàn)明顯遞減趨勢。結(jié)論內(nèi)眼術(shù)前使用左氧氟沙星滴眼液并采用生理鹽水進(jìn)行結(jié)膜囊沖洗,可有效預(yù)防眼內(nèi)感染,術(shù)前1h靜脈使用左氧氟沙星注射液可提高結(jié)膜囊細(xì)菌清除率。
[Abstract]:Objective to compare and analyze the aseptic effect of intraocular surgery with antiseptic drops combined with irrigation of conjunctival sac and systemic use of antimicrobial agents. Methods from April 2012 to February 2014, 100 eyes were randomly divided into control group (25 eyes) and experimental group (75 eyes). The experimental group was randomly divided into 3 groups. The patients in the experimental group and the control group were treated with bacterial culture of conjunctival sac before intervention. The patients in the control group were washed with normal saline, the eyes of group 1 were treated with levofloxacin, the eyes of group 2 were washed with levofloxacin combined with normal saline, and the eyes of group 3 were treated with levofloxacin. The bacterial culture of conjunctival sac was carried out on the 3rd day after treatment, and the results were analyzed by SPSS15.0 software. Results the positive rate of infection prevention was 36.0 in the control group and in the three experimental groups. The detection rate of pathogenic bacteria was 24.0 in control group, 12.0 in test 1, 8.0 in test 2 and 4.0 in test 3. The positive rate of bacterial culture in conjunctival sac showed a decreasing trend after intervention. Conclusion Levofloxacin eye drops and saline for conjunctival sac irrigation can effectively prevent intraocular infection. The bacterial clearance rate of conjunctival sac can be improved by intravenous levofloxacin injection 1 hour before operation.
