【摘要】：目的：綜合評(píng)價(jià)持續(xù)氣道正壓通氣治療（Continuous positive airwaypressure，CPAP）對(duì)阻塞性睡眠呼吸暫停低通氣綜合征（obstructive sleepapnea-hypopnea syndrome,OSAHS)患者血壓的影響。 方法：全面檢索Pubmed、Embase、Web of science、Cochrance、相關(guān)期刊論文、萬方等電子數(shù)據(jù)庫。檢索時(shí)限截止至2013年3月7日。對(duì)符合條件的隨機(jī)對(duì)照試驗(yàn)（RCT）進(jìn)行資料提取、質(zhì)量評(píng)價(jià)，進(jìn)行異質(zhì)性檢驗(yàn)，并根據(jù)血壓監(jiān)測方法不同及納入研究人群差異進(jìn)行亞組分析，采用Revman5.1軟件對(duì)各研究結(jié)果進(jìn)行數(shù)據(jù)合并，計(jì)算合并后WMD值及95%CI，對(duì)合并后WMD進(jìn)行統(tǒng)計(jì)學(xué)檢驗(yàn)。對(duì)各研究結(jié)果進(jìn)行敏感性分析，評(píng)價(jià)Meta分析結(jié)果的穩(wěn)定性。 結(jié)果：共納入10個(gè)RCT，包括1182例患者。Meta分析結(jié)果顯示：收縮壓組和舒張壓組異質(zhì)性檢驗(yàn)分別為：（P=0.26，I~2=20%）、（P=0.41，I~2=3%），因此采用固定效應(yīng)模型分別進(jìn)行合并。治療組和對(duì)照組之間有顯著統(tǒng)計(jì)學(xué)意義：（P＜0.00001）。提示CPAP治療可以有效降低OSAHS患者的血壓。將使用動(dòng)態(tài)血壓計(jì)監(jiān)測血壓的6個(gè)研究行亞組分析，異質(zhì)性檢驗(yàn)在收縮壓和舒張壓組分別為：（P=0.18，I~2=34%）、（P=0.66，I~2=0%），，因此采用固定相應(yīng)模型合并。結(jié)果顯示治療組和對(duì)照組之間有統(tǒng)計(jì)學(xué)意義：（P＜0.00001），提示CPAP治療可以有效降低OSAHS患者的血壓。最后將以高血壓患者為研究對(duì)象的4個(gè)研究進(jìn)行Meta分析，收縮壓組和舒張壓組異質(zhì)性分析顯示：（P=0.64，I~2=0%），（P=0.52，I~2=0%），亦采用固定效應(yīng)模型合并分析，治療組和對(duì)照組之間有統(tǒng)計(jì)學(xué)意義（P=0.002），結(jié)果提示CPAP治療對(duì)高血壓患者同樣可以有效降低血壓。 結(jié)論：CPAP治療可以有效降低OSAHS患者的血壓。
[Abstract]:Objective: to evaluate the effect of continuous positive airway pressure (CPAP) on blood pressure in patients with obstructive sleepapnea-hypopnea syndrome (OSAHS). Methods: Pubmedus Embase Web of Science Cochrance, full text Database of Chinese Journals, Electronic Database of Wanfang and other electronic databases were searched. The time limit for retrieval is up to March 7, 2013. Data extraction, quality evaluation, heterogeneity test and subgroup analysis were carried out according to the different blood pressure monitoring methods and the differences of the people involved in the RCTs. The Revman5.1 software was used to combine the data of the research results, the WMD value and 95% CI were calculated, and the combined WMD was tested statistically. Sensitivity analysis was carried out to evaluate the stability of Meta analysis results. Results: a total of 10 RCTs were included, including 1182 patients. Meta-analysis showed that the heterogeneity test of systolic blood pressure group and diastolic blood pressure group was 0.26% P0. 26 and 20% respectively. There was significant difference between the treatment group and the control group (P < 0.00001). The results suggest that CPAP therapy can effectively reduce the blood pressure of OSAHS patients. Six blood pressure monitoring studies using dynamic sphygmomanometer were performed subgroup analysis. The heterogeneity test in systolic blood pressure group and diastolic blood pressure group were 0. 18 and 0. 18%, respectively. The results showed that there was a significant difference between the treatment group and the control group (P < 0.00001), suggesting that CPAP treatment can effectively reduce the blood pressure of OSAHS patients. Finally, Meta analysis was carried out in 4 studies of hypertension patients. The heterogeneity analysis of systolic and diastolic blood pressure groups showed that the blood pressure was 0.64% and 0.52%, and the fixed effect model was also used to combine the analysis. There was significant difference between the treatment group and the control group. The results showed that CPAP treatment could also effectively lower blood pressure in patients with hypertension. Conclusion the blood pressure of patients with OSAHS can be effectively reduced by the treatment of 1: CPAP.
