Survivin,Bcl-2,Pml mRNA的表達及其與鼻咽癌臨床分期及預(yù)后的關(guān)系

【摘要】:目的:研究凋亡抑制基因Survivin, Bcl-2, Pml mRNA在鼻咽癌中的表達,通過采用實時熒光定量PCR方法檢測其在鼻咽癌組織中和外周血及慢性鼻咽炎組織、鼻咽癌放療后外周血中的表達,來探討Survivin, Bcl-2, Pml中單個基因及相互作用下與鼻咽癌的臨床分期及預(yù)后的相關(guān)性。 材料和方法:采用實時熒光定量PCR的方法檢測60例就診于昆明醫(yī)學(xué)院第一附屬醫(yī)院耳鼻喉科,鼻咽部取材并通過病理學(xué)確診為鼻咽癌的50例的活檢組織和外周血,慢性鼻咽炎多10例活組織,檢測Survivin, Bcl-2, Pml mRNA的表達,觀察三個基因在鼻咽癌和慢性鼻咽炎的活組織中的表達,觀察他們之間的相關(guān)性。并追蹤隨訪這些患者通過放療后的外周血中Survivin, Bcl-2, Pml的表達水平,來監(jiān)測Survivin, Bcl-2, Pml三個基因在鼻咽癌中的表達的相關(guān)性,觀察其與鼻咽癌預(yù)后的相關(guān)性。 結(jié)果:本研究中,Survivin, Bcl-2, Pml mRNA基因在鼻咽癌組織中的表達較慢性鼻咽炎組織中高,兩者差異有統(tǒng)計學(xué)意義(P0.01)。放療前的外周血中Survivin, Bcl-2, Pml mRNA基因相對表達量與放療后之間有顯著的差異(P0.01),放療前組織中Survivin, Bcl-2, Pml mRNA基因相對表達量于放療后外周血之間有顯著差異(P0.01),Ⅲ、Ⅳ期中Survivin, Bcl-2, Pml mRNA的陽性檢出率比工、II期患者之間增高,有頸淋巴結(jié)轉(zhuǎn)移的鼻咽癌中,Survivin, Bcl-2, Pml mRNA的陽性檢出率比無頸淋巴結(jié)轉(zhuǎn)移者之間增高,提示Survivin, Bcl-2, Pml基因的表達與鼻咽癌患者的臨床病程及預(yù)后有顯著關(guān)系。而Survivin, Bcl-2, Pml mRNA基因的表達與鼻咽癌患者的年齡、性別無關(guān)(P0.05)。 結(jié)論:通過檢測發(fā)現(xiàn)Survivin, Bcl-2,Pml的mRNA在鼻咽癌中的表達量與鼻咽癌的臨床分期及病理分期呈正相關(guān),并與鼻咽癌原發(fā)病灶侵犯程度、淋巴結(jié)浸潤、遠處轉(zhuǎn)移和TNMI臨Ⅰ床分期有相關(guān)性,而與患者的年齡、性別、無相關(guān)性,因此,Survivin, Bcl-2, Pml mRNA可作為研究鼻咽癌發(fā)生、發(fā)展過程及臨床分期、預(yù)后判斷的標(biāo)記。
[Abstract]:Objective: to study the expression of survivin, Bcl-2 and Pml mRNA in nasopharyngeal carcinoma (NPC), and to detect the expression of survivin, Bcl-2 and Pml mRNA in nasopharyngeal carcinoma (NPC) tissues, peripheral blood and chronic nasopharyngitis by real-time fluorescence quantitative PCR. To investigate the relationship between the clinical stage and prognosis of nasopharyngeal carcinoma (NPC) by single gene and interaction in survivin, Bcl-2, Pml. Materials and methods: Real-time fluorescence quantitative PCR was used to detect the biopsy tissues and peripheral blood of 60 cases of nasopharyngeal carcinoma (NPC) from the otolaryngology department of the first affiliated hospital of Kunming Medical College. The expression of survivin, Bcl-2 and Pml mRNA was detected in 10 cases of chronic nasopharyngitis, and the expression of three genes in the living tissues of nasopharyngeal carcinoma and chronic nasopharyngitis was observed, and the correlation between them was observed. The expression levels of survivin, Bcl-2 and Pml in peripheral blood of these patients were followed up after radiotherapy to monitor the expression of survivin, Bcl-2 and Pml in nasopharyngeal carcinoma, and to observe the correlation between survivin, Bcl-2 and Pml in the prognosis of nasopharyngeal carcinoma. Results: the expression of survivin, Bcl-2 and Pml mRNA in nasopharyngeal carcinoma was higher than that in chronic nasopharyngitis, and the difference was statistically significant (P 0.01). There were significant differences in the relative expression of survivin, Bcl-2, Pml mRNA gene in peripheral blood before radiotherapy and after radiotherapy.
