LASIK治療近視散光眼術后高階像差變化的多因素分析

【摘要】:目的探討LASIK治療近視散光眼術后高階像差變化與切削量、治療光區(qū)直徑和術后角膜后表面前凸值的相關性。方法隨機選取采用飛秒激光制瓣波前像差引導LASIK治療的近視散光患者69例(138眼),采集患者術前和術后6個月的高階像差值、角膜后表面前凸值,記錄患者的治療光區(qū)直徑和切削量,采用多元逐級回歸分析探討手術切削量、治療光區(qū)直徑和術后角膜后表面前凸值與術后高階像差的相關性。結果術中手術切削量為(96.26±25.14)μm,治療光區(qū)直徑為(6324.00±297.00)μm,術后6個月角膜后表面前凸值為(47.62±15.00)μm;術后6個月瞳孔直徑6 mm區(qū)的總體高階像差和去球差高階像差均方根值分別為(0.78±0.26)μm、(0.56±0.23)μm,術前為(0.49±0.21)μm、(0.45±0.22)μm,差異均有統(tǒng)計學意義(均為P0.01)。術后6個月瞳孔直徑6 mm區(qū)的總體高階像差和去球差高階像差均方根值與手術切削量(r=0.62、0.46,均為P0.01)、術后角膜后表面前凸值(r=0.49、0.45,均為P0.01)均呈正線性相關,與治療光區(qū)直徑(r=-0.56、-0.47,均為P0.01)均呈負線性相關。結論 LASIK治療近視散光眼術后高階像差較術前增大,術后高階像差增加與手術切削量、術后角膜后表面前凸值呈正線性相關,與治療光區(qū)直徑呈負線性相關。
[Abstract]:Objective to investigate the correlation between the changes of higher-order aberrations and the amount of ablation, the diameter of the treated light area and the postoperative corneal surface kyphosis after LASIK for myopic astigmatism. Methods 69 patients with myopic astigmatism treated by femtosecond laser wavefront aberration guided LASIK were randomly selected. The diameter of the treated light area and the amount of ablation were recorded. The correlation between the diameter of the treated light area and the postoperative corneal surface protruding value and the postoperative high order aberration was investigated by multiple stepwise regression analysis. Results the mean value of total high order aberration and high order aberration in 6 months after operation were 0.78 鹵0.26 渭 m and 0.56 鹵0.23 渭 m, respectively, and the mean square value of total high order aberration and despherical aberration were 0.78 鹵0.26 渭 m, 6324.00 鹵297.00 渭 m and 47.62 鹵15.00 渭 m, respectively, 6 months after operation, and the mean square value of high order aberration in 6 mm area of pupil diameter was 0.78 鹵0.26 渭 m, 0.56 鹵0.23 渭 m, respectively, in 6 months after operation, the mean square value of high order aberration in 6 mm area of pupil diameter was 0.78 鹵0.26 渭 m. The former was 0.49 鹵0.21) 渭 m (0.45 鹵0.22) 渭 m, the difference was statistically significant (P 0.01). The root mean square values of high order aberration and high order aberration in 6 mm area of pupil diameter 6 months after operation were positively correlated with the mean square value of high order aberration and the mean square value of high order aberration in the area of pupil diameter 6 months after operation (P 0. 01, P 0. 01). The mean square values of total high order aberration and high order aberration in 6 mm area of pupillary diameter were all positively correlated with the mean square value of the total high order aberration and the high order aberration in the area of 6 mm pupil diameter. There was a negative linear correlation between the treatment and the diameter of the light area, ru-0. 56g-0. 47 (P 0. 01). Conclusion the higher order aberration after LASIK for myopic astigmatism is larger than that before operation. The increase of high order aberration after operation is positively linear with the amount of operation and corneal surface kyphosis after operation, and it is negatively correlated with the diameter of the treated light area.
