【摘要】：目的:探討鼻顱底和鼻竇膽固醇肉芽腫的診斷與治療方法。方法:回顧性總結(jié)分析10例鼻顱底和鼻竇膽固醇肉芽腫患者的臨床資料。病變位于鼻顱底5例,位于上頜竇及上頜骨5例。CT顯示不同程度的骨質(zhì)破壞。所有患者均因保守治療無效而采取手術(shù)治療,其中8例為鼻內(nèi)鏡手術(shù),1例行額竇開窗聯(lián)合鼻內(nèi)鏡手術(shù),1例行唇齦溝切口聯(lián)合鼻內(nèi)鏡手術(shù)。結(jié)果:手術(shù)效果良好,術(shù)后無并發(fā)癥,隨訪8~40個(gè)月無復(fù)發(fā)。結(jié)論:鼻顱底和鼻竇膽固醇肉芽腫是鼻內(nèi)鏡手術(shù)的理想適應(yīng)證,其中少部分患者適合使用鼻內(nèi)鏡在輔助切口下完成手術(shù)。
[Abstract]:Objective: to investigate the diagnosis and treatment of cholesterol granuloma in nasal skull base and paranasal sinus. Methods: the clinical data of 10 patients with cholesterol granuloma in nasal skull base and paranasal sinus were retrospectively analyzed. The lesions were located in the nasocranial base in 5 cases, maxillary sinus and maxilla in 5 cases. Ct showed different degrees of bone destruction. All the patients were treated by surgery because of the ineffective conservative treatment, among them, 8 cases were treated by endoscopic sinus surgery, 1 case was treated with frontal sinus fenestration combined with nasal endoscopic surgery, and 1 case was treated by incision of lip and gingival groove combined with endoscopic surgery. Results: the effect of operation was good and there was no complication after operation. There was no recurrence after 8 ~ 40 months follow-up. Conclusion: cholesterol granuloma in the nasal skull base and sinuses is an ideal indication for endoscopic sinus surgery.
