前發(fā)聲期嗓音參數(shù)參考標(biāo)準(zhǔn)的制定及臨床應(yīng)用研究

【摘要】: 客觀全面的嗓音評估是嗓音治療的基礎(chǔ)。隨著計(jì)算機(jī)技術(shù)的發(fā)展和嗓音醫(yī)學(xué)的進(jìn)步,針對嗓音功能的主觀和客觀測量手段日漸豐富,尤其是嗓音的客觀測量技術(shù),在嗓音疾病的診斷和治療中發(fā)揮著重要作用。 本論文在國內(nèi)外嗓音客觀測量研究的基礎(chǔ)上,首次采用能對瞬態(tài)和穩(wěn)態(tài)聲波進(jìn)行提取分析的“發(fā)聲測量儀”(Phonation)為測量工具,選擇F0、Jitter、OQ. SQ、STF和AQ作為研究參數(shù),對前發(fā)聲期嗓音參數(shù)進(jìn)行了系列研究。研究內(nèi)容主要包括:前發(fā)聲期與穩(wěn)定期嗓音參數(shù)的比較,前發(fā)聲期嗓音測試語料的選擇,成人正常與病理嗓音的比較及學(xué)前兒童正常與病理嗓音的比較分析,正常成人與學(xué)前兒童前發(fā)聲期嗓音參數(shù)的特征分析,前發(fā)聲期嗓音參數(shù)參考標(biāo)準(zhǔn)的制訂以及前發(fā)聲期嗓音參數(shù)及其參考標(biāo)準(zhǔn)在臨床中的應(yīng)用。 通過本論文的實(shí)驗(yàn)研究,取得了以下階段性研究成果:1.前發(fā)聲期與穩(wěn)定期的嗓音參數(shù)存在顯著性差異:前發(fā)聲期與穩(wěn)定期之間的Jitter、OQ和SQ具有極顯著差異(p0.01), F0、STF和AQ具有顯著性差異(p0.05)。2.選擇出了前發(fā)聲期嗓音測試的最佳語料:成人為/ba/,兒童為/gu/。3.通過正常與病理嗓音的對比研究,發(fā)現(xiàn)成人及兒童正常和病理嗓音參數(shù)在前發(fā)聲期及穩(wěn)定期具有不同的特點(diǎn):成人及兒童正常和病理嗓音前發(fā)聲期參數(shù)F0、Jitter、OQ具有顯著差異(p0.05):OQ和FO正常值高于異常值,而Jitter正常值低于異常值。成人Jitter、SQ、AQ和STF四個(gè)參數(shù)在不同嗓音和發(fā)聲階段之間存在交互作用(p0.05),兒童F0、Jitter和AQ三個(gè)參數(shù)在不同嗓音和發(fā)聲階段之間存在交互作用(p0.05),其前發(fā)聲期的變化比穩(wěn)定期的變化表現(xiàn)更為明顯。學(xué)前兒童性別和年齡段之間各個(gè)前發(fā)聲期參數(shù)均無明顯差異(p0.05)。4.制訂了成人及學(xué)前兒童前發(fā)聲期嗓音參數(shù)的參考標(biāo)準(zhǔn);5.發(fā)現(xiàn)不同類型的嗓音障礙其前發(fā)聲期嗓音參數(shù)具有不同的特點(diǎn):功能性和器質(zhì)性嗓音障礙的FO值低于神經(jīng)性嗓音障礙;功能性嗓音障礙的Jitter、STF值低于器質(zhì)性和神經(jīng)性嗓音障礙;功能性嗓音障礙在AQ上卻高于器質(zhì)性和神經(jīng)性嗓音障礙,為嗓音障礙的鑒別診斷提供了新的思路和一定的參考價(jià)值。6.通過前發(fā)聲期嗓音參數(shù)及其參考標(biāo)準(zhǔn)的臨床應(yīng)用,驗(yàn)證了嗓音治療的有效性,并初步證實(shí)了前發(fā)聲期嗓音參數(shù)參考標(biāo)準(zhǔn)在臨床嗓音治療及療效監(jiān)控中的重要作用。 通過本論文的研究,證明前發(fā)聲期的嗓音參數(shù)及參考標(biāo)準(zhǔn)對于嗓音診斷和療效監(jiān)控具有重要意義,為嗓音功能的客觀測量提供了新的途徑和手段。在后繼的研究中,將拓寬研究對象的年齡和病理嗓音的類型,增加樣本量,進(jìn)一步研究前發(fā)聲期嗓音參數(shù)的生理和病理特點(diǎn),完善前發(fā)聲期嗓音參數(shù)參考標(biāo)準(zhǔn)的制訂,以更好地應(yīng)用于嗓音臨床和研究工作。
[Abstract]:Objective and comprehensive voice evaluation is the basis of voice therapy. With the development of computer technology and the progress of voice medicine, subjective and objective measurement methods for voice function are becoming more and more abundant, especially the objective voice measurement technology, which plays an important role in the diagnosis and treatment of voice diseases. Based on the objective measurement of voice at home and abroad, the phonometer (phonometer), which can extract and analyze transient and steady acoustic waves, is used as the measuring tool for the first time in this paper, F0 Jittero OQ is chosen as the measuring tool. SQSTF and AQ were used as parameters to study the voice parameters of prevocal period. The main contents of the study include: the comparison of voice parameters between pre-vocal period and stable period, the choice of voice test corpus, the comparison between normal voice and pathological voice in adults, and the comparison between normal voice and pathological voice in preschool children. The characteristic analysis of voice parameters in normal adults and pre-school children, the establishment of reference standards for voice parameters in pre-phonation period, and the application of voice parameters and reference standards in clinical practice. Through the experimental research in this paper, the following stage research results are obtained: 1. There were significant differences in voice parameters between prevocal period and stable stage: there was a significant difference between pre-vocal stage and stable stage in Jittero OQ and sq, and between F0 and AQ. There was significant difference between F0 and AQ. The best data for the pre-vocal voice test were selected: adults were / ba / r and children were / gur / r. 3. Through a comparative study of normal and pathological voices, It was found that the normal and pathological voice parameters of adults and children had different characteristics during the prevocal period and the stable period. The normal values of normal and pathological voice parameters F0 and Jittero OQ were significantly different between adults and children, and the normal values of P0.05: OQ and FO were higher than the abnormal values. The normal value of Jitter is lower than the outlier. There was interaction between the four parameters of AQ and STF in adults in different voice and vocal stage. In children, there was interaction between the three parameters of F0, Jitter and AQ in different voice and vocal stage. The change of prephonation period was higher than that of stable stage. Change performance is more obvious. There was no significant difference in the parameters of prephonation period between the sex and age groups of preschool children (p 0.05. 4). The reference standard for voice parameters of prephonation period in adults and preschool children was established. It was found that different types of voice disorders had different characteristics in the pre-phonation stage: the FO value of functional and organic voice disorders was lower than that of neurogenic voice disorders, the STF value of Jitterus of functional voice disorders was lower than that of organic and neurogenic voice disorders, and the FO values of functional and organic voice disorders were lower than those of neurogenic voice disorders. Functional voice disorders are higher in AQ than in organic and neurotic voice disorders, which provides a new idea and a certain reference value for the differential diagnosis of voice disorders. The effectiveness of voice therapy was verified by the clinical application of voice parameters and their reference standards in the prephonation period. The important role of the reference standards in the clinical voice therapy and the monitoring of curative effect was preliminarily confirmed. Through the research in this paper, it is proved that the voice parameters and reference standards in the pre-vocal period are of great significance for the diagnosis and monitoring of the curative effect of the voice, and provide a new way and means for the objective measurement of the voice function. In the subsequent studies, we will broaden the age of the subjects and the types of pathological voices, increase the sample size, further study the physiological and pathological characteristics of the voice parameters in the pre-vocal period, and improve the formulation of the reference standards for the parameters of the voice in the pre-vocal period. To better apply to voice clinical and research work.
