【摘要】：目的：觀察影響鞏膜環(huán)扎聯(lián)合加壓術(shù)后視力恢復(fù)的相關(guān)因素。 方法：選取2009年3月-2010年12月由吉林大學第二臨床醫(yī)學院眼科醫(yī)院同一術(shù)者完成環(huán)扎聯(lián)合加壓術(shù)治療RRD患者91例(91眼)的臨床資料進行回顧性分析。并采用卡方檢驗對影響術(shù)后視力恢復(fù)的相關(guān)因素進行分析。 結(jié)果：(1)91例患者年齡20歲16例,20歲-40歲44例,40歲-60歲19例,≥60歲12例。(2)主訴：視力下降51眼,視野缺損36眼,飛蚊癥16眼,視物變形13眼,閃光感8眼。(3)術(shù)前視力：術(shù)前光感3眼,手動／指數(shù)32眼,0.02-0.1者10眼,0.1-0.3者24眼,≥0.3者22眼。(4)術(shù)前屈光狀態(tài)：正視22眼,近視60眼,其中輕度近視6眼,中度近視21眼,高度近視33眼,遠視9眼。(5)視網(wǎng)膜脫離時間：1W者14眼,1W-1M者40眼,1 M-3M者23眼,3M-6M者7眼,≥6M者7眼。(6)視網(wǎng)膜脫離范圍：1個象限19眼,2個象限44眼,3個象限21眼,4個象限7眼；累及黃斑者32眼。(7)PVR分級：91眼中7眼無增殖性玻璃體視網(wǎng)膜病變,84眼存在不同程度的PVR,其中A級20眼,B級42眼,C級22眼。(8)術(shù)后視力情況：術(shù)后5天：視力提高40眼,視力不變30眼,視力下降21眼；術(shù)后3個月,視力提高71眼,視力不變11眼,視力下降9眼。(9)通過對結(jié)果行卡方檢驗發(fā)現(xiàn),不同年齡、不同視網(wǎng)膜脫離時間、不同視網(wǎng)膜脫離范圍、黃斑區(qū)累及與否及不同PVR分級的患者術(shù)后視力恢復(fù)情況有差別,且差別有統(tǒng)計學意義(P0.05)；術(shù)前不同的屈光狀態(tài),對術(shù)后視力恢復(fù)情況無差別(P0.05)。 結(jié)論：(1)RRD好發(fā)于中青年人；老年患者術(shù)后視力提高情況較年輕患者好。(2)促使RRD患者就診的最常見主訴是視力下降和視野缺損。(3)術(shù)前不同屈光狀態(tài),術(shù)后視力恢復(fù)情況無差別。(4)視網(wǎng)膜脫離時間越久,術(shù)后視力恢復(fù)越差。(5)視網(wǎng)膜脫離范圍越大,術(shù)后視力恢復(fù)越差。(6)黃斑區(qū)受累者術(shù)后5天和術(shù)后3個月視力恢復(fù)情況較黃斑未受累者視力提高率高,但是術(shù)后視力普遍偏低。(7)術(shù)前PVR越嚴重,術(shù)后視力恢復(fù)越差。
[Abstract]:Objective: to observe the factors influencing the recovery of visual acuity after scleral ligation combined with compression. Methods: from March 2009 to December 2010, 91 patients (91 eyes) with RRD were treated with ring ligation combined with compression by the same operation in ophthalmology hospital of the second College of Clinical Medicine of Jilin University. The clinical data were retrospectively analyzed. Chi-square test was used to analyze the related factors affecting the recovery of visual acuity after operation.
