【摘要】：目的探討鼻內(nèi)鏡下額竇炎手術(shù)后復(fù)發(fā)原因。方法 2009年1月~2012年6月對74名(103例/側(cè))復(fù)發(fā)性額竇炎患者行修正性鼻內(nèi)鏡手術(shù),術(shù)中探查額隱窩阻塞因素,分析額竇炎癥狀再發(fā)原因。結(jié)果共探查103例(側(cè))額隱窩,發(fā)現(xiàn)阻塞原因有以下5種:黏膜病變(67%);殘留氣房(64%);中鼻甲外移(30%);纖維瘢痕增生(14%);骨質(zhì)增生(7%)。常為多種病因共存于同一額隱窩中。結(jié)論復(fù)發(fā)性額竇炎可由多種額隱窩阻塞性因素引起,需施行修正性鼻內(nèi)鏡手術(shù)。選擇適當(dāng)?shù)氖中g(shù)方式有利于暢通額隱窩引流,避免手術(shù)失敗。
[Abstract]:Objective to investigate the causes of recurrence of frontal sinusitis after endoscopic sinus surgery. Methods from January 2009 to June 2012, 74 patients with recurrent frontal sinusitis were treated with modified endoscopic sinus surgery. The factors of frontal recess obstruction were investigated and the causes of recurrent symptoms of frontal sinusitis were analyzed. Results A total of 103 cases (lateral) of frontal recess were explored. The causes of obstruction were as follows: mucosal lesion 67%; residual chamber 64; middle turbinate external migration 30%; fibrous scar hyperplasia 14%; bone hyperplasia 7%. It is common for many causes to coexist in the same frontal recess. Conclusion recurrent frontal sinusitis may be caused by multiple obstructive factors of frontal recess. Proper surgical methods are helpful to smooth the drainage of frontal recess and avoid the failure of operation.
