【摘要】：目的探討硬皮病有眼部表現(xiàn)患者的臨床特點。設計回顧性病例系列。研究對象2008年12月至2013年2月北京同仁醫(yī)院風濕免疫科住院的13例伴眼部受累硬皮病患者。方法回顧性分析13例累及眼部的硬皮病患者的臨床資料,總結該病臨床及實驗室特點。主要指標硬皮病患者的各種眼部表現(xiàn)占總病例數(shù)的百分比。結果 13例硬皮病患者均為女性;眼部受累的平均發(fā)病年齡為(43±12.2)歲,硬皮病平均病程(6.1±5.1)年,眼病平均病程(1.4±1.9)年。系統(tǒng)型硬皮病11例(84.6%);局限型硬皮病2例(15.4%)。以眼部癥狀為首發(fā)表現(xiàn)者7例(53.8%)。干燥性角結膜炎7例(53.8%),眼瞼皮膚改變3例(23.1%)和青光眼3例(23.1%)為常見眼部表現(xiàn);其他表現(xiàn)依次為視神經(jīng)炎、角膜炎、虹睫炎(各2例,占15.4%),鞏膜炎、白內障、葡萄膜炎、中心性漿液性脈絡膜視網(wǎng)膜病變、眼球內陷、淚腺炎(各1例,占7.7%)。未發(fā)現(xiàn)眼部受累與全身表現(xiàn)及實驗室指標相關。結論各型硬皮病均可出現(xiàn)眼部受累,其中以系統(tǒng)型硬皮病為主。硬皮病眼部表現(xiàn)多樣,受累范圍廣泛,其中以干燥性角結膜炎最常見。
[Abstract]:Objective to investigate the clinical features of scleroderma with ocular manifestations. A retrospective case series was designed. 13 patients with ocular involvement in the Department of Rheumatology of Beijing Tongren Hospital from December 2008 to February 2013 were studied. Methods the clinical data of 13 patients with ocular scleroderma were analyzed retrospectively, and the clinical and actual conditions of the disease were summarized. The percentage of ocular manifestations of scleroderma patients accounted for the percentage of total cases. Results all 13 cases of scleroderma were women; the average age of onset of ocular involvement was (43 + 12.2) years, average course of scleroderma (6.1 + 5.1) years, average course of disease (1.4 + 1.9) years, systemic scleroderma 11 cases (84.6%), and localized scleroderma 2 cases. (15.4%) 7 cases (53.8%) with ocular symptoms as the first manifestation. 7 cases of dry keratoconjunctivitis (53.8%), 3 cases of eyelid skin change (23.1%) and 3 cases of glaucoma (23.1%) as common ocular manifestations; other manifestations were optic neuritis, keratitis, siphon (2 cases, 15.4%), sclera, cataract, uveitis and central serous choroidal retina. Membrane lesions, invagination, and lacrimal gland inflammation (1 cases, accounting for 7.7%). No eye involvement was found to be associated with systemic manifestations and laboratory indicators. Conclusion all types of scleroderma may be involved in eye involvement, mainly systemic scleroderma. Scleroderma has a wide range of ocular manifestations and a wide range of involvement, among which dry keratoconjunctivitis is the most common.
