糖尿病視網(wǎng)膜病變新生血管生成與血清VEGF、ES、TSP關(guān)系研究

【摘要】:目的: 探討糖尿病性視網(wǎng)膜病變(diabetic retinopathy,DR)患者血清VEGF、ES、 TSP水平及它們與眼底病變的關(guān)系,新生血管生成因子與抑制因子關(guān)系。 方法: 采用ELISA法檢測(cè)51例DR患者(觀察組)和10例無(wú)DR表現(xiàn)的糖尿病人(對(duì)照組)血清血管內(nèi)皮生長(zhǎng)因子(vascular endothelial growth factor,VEGF)、內(nèi)皮抑素(endostatin,ES)及血小板反應(yīng)蛋白(thrombospondin,TSP)水平,對(duì)比分析各指標(biāo)及其與眼底病變的關(guān)系并探討新生血管生成因子與抑制因子關(guān)系。 結(jié)果: 不同程度糖尿病性視網(wǎng)膜病變組之間VEGF有統(tǒng)計(jì)學(xué)意義(P0.01),且VEGF水平呈遞增趨勢(shì);不同程度糖尿病性視網(wǎng)膜病變組之間ES有統(tǒng)計(jì)學(xué)意義(P0.01),ES水平在對(duì)照組與非增生性糖尿病視網(wǎng)膜病變(NPDR)組病人呈遞增趨勢(shì),在增生性糖尿病視網(wǎng)膜病變(PDR)組卻顯著降低;TSP水平在不同程度糖尿病性視網(wǎng)膜病變組之間沒有統(tǒng)計(jì)學(xué)意義(P0.05)。VEGF與眼底病變程度具有顯著相關(guān)性(r=0.84,p0.05);對(duì)照組與非增生性糖尿病視網(wǎng)膜病變(NPDR)病人血清中ES與眼底病變程度具有顯著相關(guān)性(r=0.82,P0.05);TSP與眼底病變程度無(wú)相關(guān)性(r=-0.06,p0.05)。對(duì)照組+NPDR病人血清VEGF與ES具有相關(guān)性(r=0.73,p0.05);增生性糖尿病視網(wǎng)膜病變(PDR)病人血清VEGF與ES之間無(wú)相關(guān)性(r=0.01,p0.05),VEGF與TSP之間無(wú)相關(guān)性(r=-0.13,p0.05),ES與TSP之間無(wú)相關(guān)性(r=0.01,p0.05)。 結(jié)論: 血清VEGF與ES水平在非增生性糖尿病視網(wǎng)膜病變病人中處于平衡狀態(tài),而在增生性糖尿病視網(wǎng)膜病變病人時(shí)VEGF與ES之間這種平衡狀態(tài)的改變可能決定了增生性糖尿病視網(wǎng)膜病變時(shí)新生血管的發(fā)生,提示血管生成因子和血管抑制因子作用的失衡可能是增生性糖尿病視網(wǎng)膜病變新生血管形成的原因。
[Abstract]:Objective: To investigate the serum levels of VEGF ESand TSP in diabetic retinopathy and their relationship with fundus lesions and the relationship between neovascularization factor and suppressor factor. Methods: The relationship between neovascularization factor and suppressor factor was studied. Results: There was a significant difference in VEGF between different degree diabetic retinopathy group (P 0.01), and the level of VEGF showed an increasing trend. The levels of es in different degree diabetic retinopathy group were significantly higher than those in control group and non-proliferative diabetic retinopathy group. There was a significant correlation between serum es and the degree of fundus lesion in patients with diabetic retinopathy (NPDR). There was no correlation between tsp and the degree of fundus lesion. Conclusion: Serum levels of VEGF and es were balanced in patients with non-proliferative diabetic retinopathy. The change of the balance between VEGF and es in patients with proliferative diabetic retinopathy may determine the occurrence of neovascularization in proliferative diabetic retinopathy. The results suggest that the imbalance of angiogenesis and angiostatin may be the cause of angiogenesis in proliferative diabetic retinopathy.
