C反應(yīng)蛋白和血同型半胱氨酸與年齡相關(guān)性黃斑變性的相關(guān)性研究

【摘要】: 目的:探討外周靜脈血中的一些標(biāo)記物對年齡相關(guān)性黃斑變性(age-related macular degeneration,AMD)的意義。 方法:選取2009年9月至2010年4月在重慶醫(yī)科大學(xué)附屬第一醫(yī)院眼科門診就診愿意進(jìn)行研究的患者,進(jìn)行簡單調(diào)查,調(diào)查內(nèi)容包括年齡,性別,現(xiàn)病史,既往史,聯(lián)系方式等。通過裂隙燈檢查,直接眼底鏡,間接眼底鏡,光學(xué)相干斷層成像(optical coherence tomography,OCT),眼底熒光血管造影(fluorescein fundus angiography,FFA),眼底照相(fundus photography,FP)等檢查,選擇確診為AMD的患者進(jìn)入病例組(group A)。按病例對照研究方法,選取重慶醫(yī)科大學(xué)附屬第一醫(yī)院眼科住院患者中年齡50歲以上,即將手術(shù)的單純性老年性白內(nèi)障患者,作為對照組(group C)。分別抽取空腹靜脈血檢測以下11個(gè)指標(biāo): C反應(yīng)蛋白(C reactive protein ,CRP),凝血酶原時(shí)間(prothrombin time,PT),凝血酶原時(shí)間比值(prothrombin ratio ,PTR),國際標(biāo)準(zhǔn)化比值(international normalized ratio ,INR),活動(dòng)度(HGD),活化的部分凝血活酶時(shí)間(activated partial thromboplastin time,APTT),凝血酶時(shí)間(thrombin time,TT),纖維蛋白原(fibrinogen,FBG),葉酸(folic acid,FA),維生素B12(vitaminB12,VB12),血同型半胱氨酸(homocysteine,tHCY)。對數(shù)據(jù)結(jié)果采用統(tǒng)計(jì)軟件包SPSS14.0進(jìn)行統(tǒng)計(jì)分析。對病例組和對照組各個(gè)指標(biāo)比較采用兩獨(dú)立樣本均數(shù)的t檢驗(yàn),以P0.05為有統(tǒng)計(jì)學(xué)意義。 結(jié)果: 14例AMD患者CRP用均值±標(biāo)準(zhǔn)差表示為7.47±11.02(mg/l),5例對照組為6.48±4.49(mg/l),病例組與對照組無顯著性差異(P0.05)。PT:病例組11.46±1.41(sec),對照組11.46±1.41(sec),病例組與對照組無顯著性差異(P0.05)。PTR:病例組0.93±0.02,對照組0.95±0.07。病例組與對照組無顯著性差異(P0.05)。INR:病例組0.92±0.02,對照組0.95±0.07。病例組與對照組無顯著性差異(P0.05)。HGD:病例組123.05±6.56,對照組116.46±22.78。病例組與對照組無明顯差異(P0.05)。APTT:20.42±2.05(sec),對照組23.66±2.71(sec)。病例組與對照組有顯著性差異(P0.05)。TT:病例組17.34±2.73(sec),對照組16.38±0.82(sec)。病例組與對照組無顯著性差異(P0.05)。FBG:病例組3.04±0.55(g/L),對照組2.86±.652(g/L)。病例組與對照組無顯著性差異(P0.05)。FA:病例組8.30±4.10(ng/ml),對照組16.77±3.88(ng/ml)。病例組比對照組FA值低,有顯著性差異(P0.05)。VB12:病例組518.41±201.03(Pg/ml),對照組354.22±73.50(Pg/ml)。病例組與對照組無顯著性差異(P0.05)。tHCY:病例組18.43±6.37(umol/l),對照組13.30±5.19(umol/l)。病例組與對照組無顯著性差異(P0.05)。 結(jié)論: 1.本研究未發(fā)現(xiàn)CRP和tHCY對AMD的診斷有直接意義。2.AMD患者體內(nèi)葉酸含量明顯低于未患AMD的同齡人,葉酸缺乏可能是引起AMD發(fā)生的原因之一。3. AMD患者APTT明顯短于未患AMD的同齡人,可能與其血液高凝狀態(tài)有關(guān)。
[Abstract]:Objective: to investigate the significance of some markers in peripheral venous blood for age-related macular degenerationin age-related macular degeneration.Methods: from September 2009 to April 2010, the patients who were willing to study in the first affiliated Hospital of Chongqing Medical University were investigated, including age, sex, current medical history, past history, contact method and so on.Through slit lamp examination, direct fundus microscopy, indirect fundus microscopy, optical coherence tomography, fluorescein fundus angiographyFFAA, fundus photography, and so on, the patients diagnosed as AMD were selected to enter group A.According to the case-control study, simple senile cataract patients, aged over 50 years old and about to be operated on, were selected as the control group in the first affiliated Hospital of Chongqing Medical University.The following 11 indexes were detected: C-reactive protein C reactive protein, prothrombin time, prothrombin ratio, international normalized ratio, active partial thromboplastin.Activated partial thromboplastin, thrombin time, fibrinogen, folic acid, vitamin B12, homocysteine and homocysteine.The data were analyzed by SPSS14.0 software package.Case group and control group were compared with each other using the t test of the average of two independent samples, with P0.05 as the statistical significance.Results: the mean 鹵standard deviation of CRP in 14 patients with AMD was 7.47 鹵11.02 mg / L, which was 6.48 鹵4.49 mg / L in the control group. There was no significant difference between the case group and the control group (P 0.05). There was no significant difference between the case group (11.46 鹵1.41) and the control group (11.46 鹵1.41). HGD was 123.05 鹵6.56 in the case group and 116.46 鹵22.78 in the control group.There was no significant difference between the case group and the control group (P 0.05N. APTT: 20.42 鹵2.05) and the control group (23.66 鹵2.71).There was significant difference between the case group and the control group (P 0.05). TT: the case group was 17.34 鹵2.73 and the control group was 16.38 鹵0.82.There was no significant difference between the case group and the control group (P 0.05). FBG: the case group was 3.04 鹵0.55 g / L, the control group was 2.86 鹵.652 g / L.There was no significant difference between the case group and the control group (P 0.05). FA: the case group was 8.30 鹵4.10 ng / ml, and the control group was 16.77 鹵3.88 ng / ml 路ml ~ (-1).The FA value in the case group was lower than that in the control group (P 0.05). T HCY: the case group was 18.43 鹵6.37 渭 mol / L, the control group was 13.30 鹵5.19 渭 mol / L.There was no significant difference between the case group and the control group (P 0.05).Conclusion: 1.In this study, we found no direct significance of CRP and tHCY in the diagnosis of AMD. 2. The level of folic acid in patients with CRP is significantly lower than that of their peers without AMD. Folic acid deficiency may be one of the causes of AMD.The APTT of AMD patients was significantly shorter than that of their peers without AMD, which may be related to their blood hypercoagulability.
