光學(xué)相干斷層掃描儀在慢性原發(fā)性閉角型青光眼早期診斷中的應(yīng)用

【摘要】: 目的 探討光學(xué)相干斷層掃描儀(optical coherence tomography,OCT)檢測視網(wǎng)膜神經(jīng)纖維層(retinal nerve fiber layer,RNFL)厚度在慢性原發(fā)性閉角型青光眼(Chronic primary angle-closure glaucoma, CPACG)早期診斷中應(yīng)用。 方法 正常對照組19例(28眼),青光眼患者30例(39眼)并分為早期、中晚期2組。采用OCT進(jìn)行盤周RNFL厚度的測量,比較正常對照組與早期CPACG患者RNFL厚度的差異,并通過計(jì)算RNFL各厚度參數(shù)在ROC曲線下面積評價OCT在青光眼早期診斷中的應(yīng)用。 結(jié)果 早期CPACG患者平均和顳側(cè)RNFL厚度及視盤周圍1點(diǎn)、7點(diǎn)、9點(diǎn)、10點(diǎn)、11點(diǎn)鐘RNFL厚度較正常對照組明顯變。怀3點(diǎn)鐘外,早期及中晚期CPACG的RNFL厚度較正常對照組明顯變。9點(diǎn)、10點(diǎn)及顳側(cè)RNFL在ROC曲線下面積最大,分別為0.819、0.816、0.811。結(jié)論 早期CPACG的RNFL已變薄,通過計(jì)算早期CPACG厚度參數(shù)在ROC曲線下面積可以為青光眼的早期診斷提供了一項(xiàng)有用指標(biāo)。
[Abstract]:PurposeObjective to investigate the application of optical coherence tomography (coherence) in the early diagnosis of chronic primary angle closure glaucoma (CPACG) by using optical coherence tomography (coherence) to detect the thickness of retinal nerve fiber layer (RNFL) in chronic primary angle-closure glaucoma.MethodThe normal control group consisted of 19 patients (28 eyes) and 30 glaucoma patients (39 eyes).The peridisc RNFL thickness was measured by OCT, and the difference of RNFL thickness between the normal control group and the early CPACG patients was compared, and the application of OCT in the early diagnosis of glaucoma was evaluated by calculating the thickness parameters of RNFL under the ROC curve.ResultThe mean and temporal RNFL thickness in early CPACG patients and the RNFL thickness at 10 and 11 o'clock around the optic disc were significantly thinner than those in the normal control group, except at 3 o'clock.The RNFL thickness of early and middle stage CPACG was significantly thinner than that of normal control group at 9 points and 10 points, and the area of temporal RNFL was the largest under the ROC curve, which was 0.819 ~ 0.816 ~ 0.811, respectively.ConclusionThe RNFL of early CPACG has been thinned. Calculating the area under ROC curve of early CPACG thickness parameter can provide a useful index for the early diagnosis of glaucoma.
