【摘要】：目的總結(jié)眼眶內(nèi)腫瘤和眶-顱溝通腫瘤經(jīng)顱手術(shù)治療方法和手術(shù)技巧。方法回顧分析經(jīng)顱手術(shù)治療的眼眶內(nèi)腫瘤和眶-顱溝通腫瘤患者22例的臨床表現(xiàn)、手術(shù)方法及療效。其中,眼眶內(nèi)腫瘤9例,眶-顱溝通腫瘤13例。均以翼點(diǎn)入路為基礎(chǔ),再根據(jù)具體情況擴(kuò)大骨窗范圍。結(jié)果病灶全切18例,次全切除3例,大部切除1例。術(shù)后動(dòng)眼神經(jīng)損傷4例,視力下降或失明4例,發(fā)熱和腦脊液漏各1例。無(wú)手術(shù)死亡病例。隨訪4-48個(gè)月;除3例失訪外,隨訪期內(nèi)僅1例患者腫瘤復(fù)發(fā)二次手術(shù)。結(jié)論根據(jù)病灶的具體情況,采取相應(yīng)的手術(shù)入路,嚴(yán)密保護(hù)視神經(jīng),實(shí)施經(jīng)顱眼眶內(nèi)腫瘤和眶-顱溝通腫瘤手術(shù)可以在最小創(chuàng)傷下,有效切除病灶。
[Abstract]:Objective to summarize the methods and techniques of transcranial surgery for orbital tumors and orbital-cranial communication tumors.Methods 22 cases of orbital tumor and orbital-cranial communication tumor treated by transcranial surgery were retrospectively analyzed.There were 9 cases of orbital tumors and 13 cases of orbital-cranial communication tumors.Based on the pterygoid approach, the bone window was expanded according to the specific conditions.Results Total resection was performed in 18 cases, subtotal resection in 3 cases and subtotal resection in 1 case.There were 4 cases of oculomotor nerve injury, 4 cases of vision loss or blindness, 1 case of fever and 1 case of cerebrospinal fluid leakage.There were no surgical deaths.The patients were followed up for 4 to 48 months, except for 3 cases who lost the visit, only one patient underwent secondary operation during the follow-up period.Conclusion according to the specific condition of the lesion, the operative approach should be adopted, the optic nerve should be protected closely, the transcranial orbital tumor and the orbito-cranial communication tumor could be excised effectively under the minimal trauma.
