【摘要】：正患者,男,50歲,因"間斷右鼻出血2d"入院。入院后,前鼻鏡檢查未見出血部位,咽后壁可見活動性血流,且患者精神緊張,不能配合鼻內(nèi)鏡檢查,遂局部麻醉下行右側后鼻孔栓塞加右前鼻孔填塞,填塞后無活動性滲血。既往史:飲酒史35年,早、中、晚餐均飲酒(酒精度60%~70%vol),每天飲酒總量在500ml左右。入院后查血常規(guī)、出凝血時間、肝腎功能均未見異常,予抗炎、止血對癥治療。擬72h后開始逐步抽取前鼻孔填塞紗條。第3天晨查房時,患者訴"右側后鼻孔有流水聲,持續(xù)時間約10s,發(fā)作較頻繁",并強調一直能聽見該聲音。
[Abstract]:The patient, 50 years old, was admitted to hospital due to intermittent right epistaxis for 2 days.Past history: 35 years of drinking history, early, middle, and dinner drinking (alcohol 60%, 70%), the total amount of daily drinking is about 500ml.Check blood routine after admission, coagulation time, liver and kidney function were not abnormal, anti-inflammatory, hemostatic treatment.After 72 hours, the packing gauze of the anterior nostril was gradually extracted.On the third morning, the patient complained of "flowing water sound in the right posterior nostril, lasting about 10 s, with frequent seizures," and stressed that the sound could be heard all the time.
