光學(xué)相干斷層成像術(shù)在老年開角型青光眼視網(wǎng)膜神經(jīng)纖維層厚度檢測中的應(yīng)用價值

【摘要】:目的探討光學(xué)相干斷層成像術(shù)(OCT)在老年開角型青光眼(POAG)視網(wǎng)膜神經(jīng)纖維層厚度檢測中的應(yīng)用價值。方法選擇初診30例老年P(guān)OAG患者為觀察組,并根據(jù)病情的早期、進展期、晚期分為觀察1組,2組,3組,另取30例正常人群作為對照組,予以所有研究對象常規(guī)眼科檢查以及OCT檢查,觀察組進行視野檢查。結(jié)果與對照組比較,觀察1、2、3組的RNFL厚度測量中上方、下方、顳側(cè)、鼻側(cè)以及均值均降低明顯(P0.05);觀察組組間比較,與觀察1組比較,觀察2、3組的各項指標(biāo)均降低明顯(P0.05)。就OCT圖像而言,對照組可見上、下極厚,其次為顳側(cè),鼻側(cè)最薄,圖像曲線表現(xiàn)為雙峰形;POAG患者RNFL的早期可表現(xiàn)局限性的變薄或缺損,進展期則表現(xiàn)彌漫性變薄,晚期則表現(xiàn)為彌漫性變薄,且混合局限性缺損,視野檢測的缺損均值與RNFL厚度呈負(fù)相關(guān),尤以RNFL平均厚度相關(guān)性最大。結(jié)論 OCT以其可較好地區(qū)分RNFL厚度而用于老年P(guān)OAG的診斷之中,且隨著病程的發(fā)展,RNFL厚度逐漸變薄,視野的平均缺損范圍則漸趨增加。
[Abstract]:Objective to evaluate the value of optical coherence tomography (Oct) in detecting the thickness of retinal nerve fiber layer (RNFL) in aged patients with open-angle glaucoma (POAGG).Methods A total of 30 elderly POAG patients were selected as the observation group and divided into three groups according to the early stage and advanced stage of the disease. Another 30 normal persons were taken as the control group.All subjects were given routine ophthalmologic examination and OCT examination, and the observation group underwent visual field examination.Results compared with the control group, the thickness of RNFL was significantly decreased in the upper, lower, lower temporal, nasal and average sides of the three groups, and compared with the control group, all the indexes of the observation group were significantly lower than those of the observation group 1 (P 0.05), and that of the control group was significantly lower than that of the control group (P < 0.05), while that of the control group was significantly lower than that of the control group (P < 0.05).As far as OCT images were concerned, in the control group, the upper and lower sides were thicker, the temporal side was the second, the nasal side was the thinnest. The image curve showed that the RNFL of the patients with bimodal Poag showed local thinning or defect at the early stage, and diffuse thinning at the advanced stage.In the later stage, diffuse thinning and mixed local defect were observed. The mean value of visual field defect was negatively correlated with the thickness of RNFL, especially the average thickness of RNFL.Conclusion OCT can be used in the diagnosis of senile POAG because it can distinguish the thickness of RNFL well. With the development of the course of disease, the thickness of OCT becomes thinner, and the average defect area of visual field increases gradually.
