【摘要】：目的： 1.建立青少年開角型青光眼（Juvenile-Onset open-angle glaucoma,JOAG）遺傳家系臨床信息和數(shù)字檔案資料及DNA標(biāo)本庫。 2.分析青少年開角型青光眼遺傳家系臨床表型和遺傳學(xué)特征。 3.鑒定引起青少年開角型青光眼遺傳家系致病基因突變位點(diǎn)。 方法： 1.收集一個(gè)4代共14人的青少年開角型青光眼遺傳家系成員病史及臨床資料，簽署臨床調(diào)查及科研知情同意書，填寫JOAG調(diào)查表，繪制家系圖譜。抽取家系內(nèi)12個(gè)成員外周血8～10ml并進(jìn)行了系統(tǒng)的眼科檢查，包括矯正視力，眼壓測量，角膜中央?yún)^(qū)厚度測量，房角檢查，裂隙燈檢查，眼底檢查、視野檢查和視盤及黃斑OCT檢查。 2.應(yīng)用聚合酶鏈反應(yīng)（polymeasreehaineraction, PCR)和Sanger測序法，對該家系12名成員4個(gè)候選基因（CYP1B1, MYOC, OPTN和WDR36基因）所有外顯子區(qū)域進(jìn)行致病基因突變位點(diǎn)篩查，結(jié)果與NCBI的dbSNP數(shù)據(jù)庫、千人數(shù)據(jù)庫、ensemble數(shù)據(jù)庫等進(jìn)行比對，并且將該突變位點(diǎn)在家系內(nèi)無親緣關(guān)系的正常人和100個(gè)家系之外的對照組中進(jìn)行驗(yàn)證。 結(jié)果： 1.建立了科學(xué)、規(guī)范，，符合遺傳資源采集研究原則要求的GD-JY001遺傳家系臨床資料和數(shù)字檔案數(shù)據(jù)庫及DNA標(biāo)本庫。 2.該家系4代共14名成員,現(xiàn)存活患者12名。5個(gè)患者發(fā)病年齡平均14.4歲（11-16歲），平均眼壓44.2mmHg（28-60mmHg），視神經(jīng)出現(xiàn)不對稱性改變和視野損害�；颊叩姆拷菣z查顯示小梁網(wǎng)有更加多的色素沉積和虹膜根粗大形成房角粘連。家族中大多患者帶有一個(gè)較薄的中心角膜厚度（central corneathickness，CCT），患者的平均CCT為488.625mm（529-446mm）。 3. GD-JY001家系所有患者在MYOC基因第3號外顯子編碼區(qū)發(fā)現(xiàn)雜合錯(cuò)義突變c.733T＞G，p.Cys245Gly，該突變與GD-JY001家系表型共分離。在NCBI的dbSNP數(shù)據(jù)庫、千人數(shù)據(jù)庫、ensemble數(shù)據(jù)庫以及www.myocilin.com網(wǎng)站中均未發(fā)現(xiàn)有該位點(diǎn)改變的報(bào)道，并且在100個(gè)無親緣關(guān)系的正常人中不存在該突變位點(diǎn)。測序結(jié)果在CYP1B1, OPTN和WDR36基因的外顯子區(qū)未發(fā)現(xiàn)致病突變。 結(jié)論： 1.青少年開角型青光眼GD-JY001家系遺傳方式符合常染色體顯性遺傳特征�；颊吲R床表型為發(fā)病時(shí)間早，眼壓升高顯著，眼底視神經(jīng)損害呈不對稱性改變，視野損害與眼底改變一致。所有患者有異常的房角改變包括房角色素增加和虹膜根異常增粗并與房角形成粘連。 2.雜合錯(cuò)義突變c.733T＞G，p.Cys245Gly是青少年開角型青光眼GD-JY001家系致病基因突變，其候選基因CYP1B1, OPTN和WDR36與該家系的致病基因無相關(guān)性。
[Abstract]:Objective:. 1. To establish the pedigrees clinical information, digital file data and DNA specimen library of juvenile open angle glaucoma (juvenile open angle glaucoma). 2. To analyze the clinical phenotype and genetic characteristics of pedigrees with juvenile open angle glaucoma. 3.Identification of mutation locus of pathogenic gene in pedigrees of pedigrees causing juvenile open-angle glaucoma. Methods:. 1. To collect the medical history and clinical data of a family of 14 adolescents with open angle glaucoma for 4 generations, sign the consent form of clinical investigation and scientific research, and fill out the JOAG questionnaire. Drawing pedigree map. Peripheral blood 8~10ml was extracted from 12 members of the family and systematic ophthalmic examination was carried out, including corrected visual acuity, intraocular pressure measurement, corneal central area thickness measurement, atrial angle examination, slit lamp examination, fundus examination. Visual field examination and OCT examination of optic disc and macula. 2. Polymerase chain reaction (PCR) and Sanger sequencing were used to screen all exons of CYP1B1, MYOC, OPTN and WDR36 gene. The results were compared with the dbSNP database of NCBI. The database of thousands of people was compared, and the mutation site was verified in the normal persons who were not related to each other in the family and the control group outside 100 families. Results:. 1. The GD-JY001 genealogy clinical data database, digital file database and DNA specimen database were established, which were scientific, standardized and in accordance with the principles of genetic resources collection and research. 2. The family consists of 14 members for four generations, There were 12 surviving patients. The mean age of onset of 5 patients was 14.4 years old and 11-16 years old, the mean IOP was 44.2 mm HgGG 28-60 mm HgG, the optic nerve was asymmetrical and the visual field was damaged. The atrial angle examination of the patients showed more pigmentation of trabecular meshwork and thicker iris root. Most patients in the family had a thin central corneal thickness (CCT) with an average CCT of 488.625mm (529-446mm). 3. The heterozygous missense mutation c.733T was found in all patients of GD-JY001 pedigree in the coding region of exon 3 of MYOC gene. The mutation was coisolated from the phenotype of GD-JY001 pedigree, and was isolated from the dbSNP database of NCBI. No change of the locus was reported in the thousands of human databases and www.myocilin.com sites, and the mutation was not found in 100 unrelated normal individuals. No mutations were found in the exon region of CYP1B1, OPTN and WDR36 genes. Conclusion:. 1. The genetic pattern of GD-JY001 pedigrees in juvenile open-angle glaucoma was autosomal dominant. The clinical phenotypes were early onset, significant elevation of intraocular pressure and asymmetric changes of optic nerve damage in the fundus. All patients had abnormal atrial angle changes including increased atrial role factor and abnormal thickening of iris root and formed adhesion with the angle of atrium. 2. The heterozygous missense mutation c.733T > GPp.Cys245Gly is a mutation of GD-JY001 gene in the pedigrees of juvenile open-angle glaucoma. The candidate genes CYP1B1, OPTN and WDR36 are not correlated with the pathogenic genes of this family.
