【摘要】：目的探討術(shù)前經(jīng)顱多普勒超聲(TCD)、CT血管造影(CTA)及術(shù)中微血管多普勒超聲(MVD)對(duì)于床突旁頸內(nèi)動(dòng)脈眼動(dòng)脈段動(dòng)脈瘤手術(shù)的輔助價(jià)值。 方法回顧性分析我院2009年7月到2012年6月共17例床突旁頸內(nèi)動(dòng)脈眼動(dòng)脈段動(dòng)脈瘤臨床資料,其中男5例,女12例,術(shù)前行完善的TCD及CTA檢查,經(jīng)翼點(diǎn)入路開(kāi)顱行動(dòng)脈瘤夾閉術(shù),術(shù)中使用MVD輔助動(dòng)脈瘤夾閉。 結(jié)果術(shù)后隨訪3-29月,平均14.3月,17例患者中,16例均獲得滿意療效,1例留有輕偏癱,術(shù)后行CTA和(或)DSA復(fù)查均示動(dòng)脈瘤夾閉完全,載瘤動(dòng)脈通暢。 結(jié)論完善的TCD及CTA檢查能提高手術(shù)的成功率,避免盲目探查,增加手術(shù)的安全性。而術(shù)中MVD的監(jiān)測(cè)能指導(dǎo)動(dòng)脈瘤的夾閉,減少患者致死、致殘率,改善患者預(yù)后。
[Abstract]:Objective to evaluate the value of preoperative transcranial Doppler echocardiography (TCD-CT) and intraoperative microvessel Doppler ultrasonography (MVD) in the operation of segmental carotid artery aneurysms in parastoid internal carotid artery. Methods from July 2009 to June 2012, 17 patients (5 males and 12 females) with ophthalmic artery aneurysms of internal carotid artery beside the clinoid process were examined by TCD and CTA. Aneurysm clipping was performed via pterygotomy and MVD assisted aneurysm clipping was used. Results all the 17 patients were followed up for 3 to 29 months, 16 of them had satisfactory curative effect and 1 had hemiplegia. The results of CTA and / or DSA showed that the aneurysm was completely clipped and the aneurysm was unobstructed. Conclusion TCD and CTA can improve the success rate of operation, avoid blind exploration and increase the safety of operation, while the monitoring of MVD during operation can guide the occlusion of aneurysms, reduce the death rate and disability rate of patients, and improve the prognosis of the patients.
