【摘要】：目的:系統(tǒng)評價(jià)巴曲酶治療突發(fā)性耳聾的效果及安全性。方法:按照Cochrane系統(tǒng)評價(jià)制作方法,計(jì)算機(jī)檢索Medline(1980-2009.9),Embase(1980-2009.9),中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(1980-2009.9)CNKI(1980-2009.9),Cochrane協(xié)作網(wǎng)耳鼻喉疾病組(2009年3期);并手工檢索相關(guān)領(lǐng)域文獻(xiàn),收集所有相關(guān)隨機(jī)對照試驗(yàn)(RCT)和半隨機(jī)對照試驗(yàn)(qasy-RCT),評價(jià)納入研究的方法學(xué)質(zhì)量,提取資料,并用Cochrane協(xié)作網(wǎng)提供的RevMan4.2軟件對數(shù)據(jù)進(jìn)行Meta分析。結(jié)果:共納入3篇隨機(jī)對照試驗(yàn),包括242例患者。Meta分析結(jié)果顯示,在使用藥物治療突發(fā)性耳聾的患者中,巴曲酶與常規(guī)藥物在治療突發(fā)性耳聾的治愈率[RR=1.79,95%CI(1.16-2.74),P=0.008]、無效率[RR=0.4,95%CI(0.25-0.64),P=0.0001]方面有統(tǒng)計(jì)學(xué)意義。在顯著有效率[RR=1.41,95%CI(0.83-2.40),P=0.2]、有效率[RR=1.03,95%CI(0.71-1.52),P=0.86]方面無統(tǒng)計(jì)學(xué)意義,與疏血通相比在各結(jié)局指標(biāo)中均無統(tǒng)計(jì)學(xué)意義。結(jié)論:巴曲酶與常規(guī)藥物在治療突發(fā)性耳聾的患者中顯著有效率、有效率無差異,但與常規(guī)藥物相比較,巴曲酶能提高患者的治愈率,降低患者的無效率,巴曲酶與疏血通的治療效果無明顯差異。由于本系統(tǒng)評價(jià)納入的研究較少,臨床研究質(zhì)量普遍偏低,且缺乏安全性資料,對巴曲酶治療突發(fā)性耳聾的有效性和安全性上不能得出肯定結(jié)論,有待于高質(zhì)量、大樣本、多中心的雙盲隨機(jī)對照試驗(yàn)結(jié)果證實(shí),以增強(qiáng)論證強(qiáng)度。
[Abstract]:Objective: to evaluate the efficacy and safety of batroxobin in the treatment of sudden deafness. Computer search of Medline #en0# (1980-2009.9) Embase 1980-2009, Chinese Biomedical Literature Database 1980-2009.9CNKIN 1980-2009.9 Cochrane Cooperative Network for Otolaryngology (issue 3, 2009), and manual retrieval of related literature. All relevant randomized controlled trials (RCTs) and semi-randomized controlled trials were collected to evaluate the quality of methodology included in the study, to extract data, and to analyze the data by Meta using RevMan4.2 software provided by the Cochrane Cooperative Network. Results: three randomized controlled trials were conducted. The results of the Meta-analysis, including 242 patients, showed that in patients with sudden deafness treated with drugs, Batroxobin and conventional drugs have statistical significance in the cure rate of sudden deafness [RRRN 1.79-95 CIQ 1.16-2.74 P0. 008], the inefficiency rate [RRN 0.495 CI 0.25-0.64 P0. 0001], but there is no statistical significance in the significant effective rate [RR1.4195CI0.83-2.40P0. 2], but there is no statistical significance in the effective rate [RR1. 0395CII 1. 735 CIQ 1. 52P 0. 86], and there is no statistical significance in the treatment of sudden deafness with Batroxobin and conventional drugs. The results show that there is no significant difference in the cure rate between Batroxobin and conventional drugs in the treatment of sudden deafness. Compared with Shuxuetong, there was no statistical significance in each outcome index. Conclusion: Batroxobin and conventional drugs have significant effective rate in the treatment of sudden deafness, but there is no difference in effective rate, but it is compared with conventional drugs. Batroxobin can improve the cure rate and reduce the inefficiency of patients. There is no significant difference in the therapeutic effect between batroxobin and Shuxuetong. The efficacy and safety of batroxobin in the treatment of sudden deafness can not be confirmed. The results of double-blind randomized controlled trials with high quality, large sample and multiple centers are needed to enhance the intensity of demonstration.
