【摘要】：目的:評(píng)估雙側(cè)先天性小耳畸形并外耳道閉鎖嬰幼兒佩戴軟帶Ponto的療效及聽(tīng)覺(jué)發(fā)育情況。方法:雙側(cè)先天性小耳畸形并外耳道閉鎖患兒20例(年齡3~21個(gè)月),聽(tīng)性腦干反應(yīng)(ABR)骨導(dǎo)反應(yīng)閾均小于25dBnHL,氣導(dǎo)反應(yīng)閾大于45dBnHL。佩戴軟帶Ponto,每天佩戴2.5~8.0h,隨訪(fǎng)3~6個(gè)月。通過(guò)視覺(jué)強(qiáng)化測(cè)聽(tīng)(VRA)評(píng)估1歲以上患兒佩戴軟帶Ponto后行為反應(yīng)閾。應(yīng)用嬰幼兒有意義聽(tīng)覺(jué)整合量表(IT-MAIS)評(píng)估其佩戴當(dāng)日、佩戴3個(gè)月及佩戴6個(gè)月時(shí)的聽(tīng)覺(jué)發(fā)育情況。結(jié)果:患兒ABR骨導(dǎo)反應(yīng)閾平均值為(17.5±5.9)dBnHL,氣導(dǎo)反應(yīng)閾平均值為(72.5±9.3)dBnHL。其中5例年齡滿(mǎn)1歲患兒佩戴軟帶Ponto后測(cè)VRA500~4 000Hz平均聽(tīng)閾為(30.5±5.9)dB HL。患兒佩戴軟帶Ponto前聽(tīng)覺(jué)發(fā)育均低于正常水平,佩戴3個(gè)月及6個(gè)月后IT-MAIS評(píng)分顯著提高,且逐漸接近正常水平。結(jié)論:軟帶Ponto有助于雙側(cè)先天性耳廓畸形合并外耳道閉鎖嬰幼兒的聽(tīng)覺(jué)發(fā)育,可作為手術(shù)植入助聽(tīng)裝置前的過(guò)渡治療,盡早應(yīng)用于臨床。
[Abstract]:Objective: to evaluate the effect and auditory development of soft band Ponto on infants with bilateral congenital microauricular malformation and atresia of external auditory meatus. Methods: 20 cases (age 3 ~ 21 months) of bilateral congenital microauricular malformation with atresia of external auditory canal were studied. Brain-stem response threshold of ABR was less than 25dBnHLand that of air-conduction was higher than 45dBnHL.The soft band Ponto was worn for 2.5 ~ 8.0 hours a day, followed up for 3 ~ 6 months. Visual intensive audiometry was used to evaluate the behavioral response threshold after wearing soft band Ponto in children over 1 year old. IT-MAIS (meaningful auditory Integration scale) was used to assess the day on which the child was wearing it. Results: the average of ABR bone conduction response threshold was 17.5 鹵5.9 dBnHLand the average of air-conduction response threshold was 72.5 鹵9.3 dBnHL.The VRA500~4 000Hz hearing threshold was measured in 5 children over 1 year old after wearing soft band Ponto. In order to be 30.5 鹵5.9 dB HL, the auditory development of children with soft band Ponto was lower than that of normal children. After 3 and 6 months of wear, the IT-MAIS score increased significantly and gradually approached the normal level. Conclusion: soft band Ponto is helpful to the auditory development of infants with bilateral congenital auricle malformation and atresia of external auditory meatus. It can be used as a transitional therapy before implantation of hearing aid device, and be used in clinic as soon as possible.
