【摘要】：目的比較選配助聽(tīng)器與人工耳蝸植入術(shù)后聽(tīng)障兒童的聽(tīng)覺(jué)言語(yǔ)康復(fù)效果,探討不同聽(tīng)覺(jué)補(bǔ)償方式對(duì)其聽(tīng)覺(jué)言語(yǔ)康復(fù)效果的影響。方法選取32例6~43月齡選配助聽(tīng)器或人工耳蝸植入后的重度及極重度感音神經(jīng)性語(yǔ)前聾患兒為研究對(duì)象,其中助聽(tīng)器組18名,人工耳蝸組14名,兩組的聽(tīng)力損失程度無(wú)統(tǒng)計(jì)學(xué)差異,均在干預(yù)1年內(nèi)進(jìn)行相同的聽(tīng)覺(jué)言語(yǔ)康復(fù)訓(xùn)練,使用聲場(chǎng)測(cè)聽(tīng)測(cè)試助聽(tīng)聽(tīng)閾;使用嬰幼兒有意義聽(tīng)覺(jué)整合量表評(píng)估干預(yù)前及干預(yù)后3、6、9、12個(gè)月時(shí)的聽(tīng)覺(jué)能力;采用《聽(tīng)障兒童聽(tīng)覺(jué)言語(yǔ)能力評(píng)估》測(cè)試兩組康復(fù)訓(xùn)練前后的言語(yǔ)能力。結(jié)果兩組患兒助聽(tīng)聽(tīng)閾均優(yōu)于助聽(tīng)前,但人工耳蝸組各頻率助聽(tīng)聽(tīng)閾顯著優(yōu)于助聽(tīng)器組(P0.05);兩組聽(tīng)覺(jué)能力得分均隨聽(tīng)覺(jué)補(bǔ)償時(shí)間的延長(zhǎng)而提高(P0.05);助聽(tīng)器組在干預(yù)后9個(gè)月的聽(tīng)覺(jué)能力得分明顯高于干預(yù)前、干預(yù)3個(gè)月及干預(yù)6個(gè)月(P0.05);人工耳蝸組在術(shù)后6個(gè)月的聽(tīng)覺(jué)能力得分高于術(shù)前及術(shù)后3個(gè)月(P0.05);人工耳蝸組在干預(yù)后6、9、12個(gè)月的聽(tīng)覺(jué)能力得分均高于助聽(tīng)器組(P0.05);兩組助聽(tīng)后康復(fù)級(jí)別及言語(yǔ)年齡均高于助聽(tīng)前,人工耳蝸組康復(fù)級(jí)別及言語(yǔ)年齡均高于助聽(tīng)器組(P0.05)。結(jié)論對(duì)于極重度以上感音神經(jīng)性聾嬰幼兒,人工耳蝸植入者聽(tīng)覺(jué)言語(yǔ)康復(fù)效果優(yōu)于助聽(tīng)器選配者。
[Abstract]:Objective to compare the effects of hearing and speech rehabilitation in children with hearing impairment after selective hearing aid and cochlear implantation. To explore the effect of different ways of auditory compensation on the recovery of hearing and speech. Methods 32 children with severe or extremely severe sensorineural hearing loss after hearing aid or cochlear implantation were selected as subjects. There were 18 hearing aids and 14 cochlear implants. There was no significant difference in hearing loss between the two groups. The auditory ability of infants was evaluated with meaningful auditory integration scale before and 3 months after intervention. The speech ability of the two groups before and after rehabilitation training was tested. Results the hearing aid threshold of the two groups was better than that before hearing aid. However, the hearing threshold of cochlear implant group was significantly better than that of hearing aid group (P 0.05), the score of hearing ability in both groups increased with the prolongation of auditory compensation time, and the hearing ability score in hearing aid group was significantly higher than that before intervention at 9 months after intervention. The score of auditory ability in cochlear group was higher than that in preoperation and 3 months after operation, and the score of auditory ability in cochlear implant group was higher than that in hearing aid group in 6 months and 12 months after intervention, and the score of auditory ability in cochlear implant group was higher than that in hearing aid group at 6 months and 6 months after intervention, and the score of auditory ability in cochlear implant group was higher than that in preoperation and 3 months after operation. The rehabilitation grade and speech age after hearing aid were higher than those before hearing aid. The rehabilitation grade and speech age of cochlear implant group were higher than that of hearing aid group P0.050.Conclusion for infants with extremely severe sensorineural hearing loss, cochlear implants have better effects on auditory speech rehabilitation than on hearing aid selection.
