【摘要】：目的:探討聯(lián)合手術治療嚴重的持續(xù)性變應性鼻炎的療效。 方法:對34例嚴重的持續(xù)性變應性鼻炎患者采用鼻中隔粘膜剝離或鼻中隔粘膜下矯正、前組篩房開放、下鼻甲部分切除的聯(lián)合手術方法,所有病例均隨訪12個月,記錄術后鼻部癥狀和體征的改善情況,并對術前及術后3個月、6個月、12個月的療效采用兩種方法進行總體癥狀的評估以及分類癥狀的評估。 結果:采用2004年蘭州《變應性鼻炎的診治原則和推薦》標準評估,術后3個月時顯效率76.5%(26/34),有效率17.7%(6/34),無效率5.9%(2/34);術后6個月時顯效率67.6%(23/34),有效率26.5%(9/34),無效率5.9%(2/34),術后12月時顯效率64.7%(22/34),有效率23.5%(8/34),無效率11.8%(4/34)。術后3個月、術后6個月,術后12個月的總的有效率分別為94.2%、94.2%、88.2%。采用視覺模擬評分法評估,本組病例術后3個月、術后6個月、術后12個月與術前的總體癥狀及分類癥狀相比,差異均P0.05,差異具有統(tǒng)計學意義。 結論:1、聯(lián)合手術是藥物治療無效的變應性鼻炎的一種有效的治療方式。 2、在嚴格掌握手術適應癥的前提下,鼻中隔粘膜分離或鼻中隔粘膜下矯正術,前組篩房開放,下鼻甲部分切除的聯(lián)合手術方式取得顯著的近期療效。 3、本手術方法簡單,在局部麻醉下進行,不需特殊的設備,有一定臨床經(jīng)驗的醫(yī)生即可施行,不需特殊的人員培訓,可廣泛地開展。 4、病人依從性好,無需長期隨診。
[Abstract]:Objective: to investigate the effect of combined operation on severe persistent allergic rhinitis. Methods: 34 patients with severe persistent allergic rhinitis were treated with combined operation of nasal septum mucosa dissection or submucosal correction, anterior ethmoid chamber opened and partial inferior turbinate resected. All patients were followed up for 12 months. The improvement of nasal symptoms and signs were recorded, and the overall symptoms and classified symptoms were evaluated by two methods before operation and 3 months, 6 months and 12 months after operation. Results: the criteria for diagnosis and treatment of allergic rhinitis in Lanzhou on 2004 were evaluated. At the third month after surgery, the effective rate was 76.5B / 26 / 34, the effective rate was 17.775 / 34, and the effective rate was 5.93.34%, and the inefficiency was 5.93.34%; at the sixth month after the operation, the significant efficiency was 67.66% / 34%, and the effective rate was 26.5m / 934%, and the inefficiency was 5.9% / 34%, and on December, the apparent efficiency was 64.775% 2234%, and the effective rate was 23.585% / 34%, and the inefficiency was 11.88.434%. Three months after surgery, six months after the operation, six months after surgery, the effective rate was 64.722% 34%, and the effective rate was 23.585% / 34%, 11.88% -434%. The total effective rate of 12 months after operation was 94.2 and 94.2respectively. The visual analogue scoring method was used to evaluate the overall symptoms and classification symptoms of the patients at 3 months, 6 months and 12 months after operation respectively. The difference was all P 0.05, the difference was statistically significant. Conclusion: combined surgery is an effective treatment for ineffective allergic rhinitis. 2. On the premise of strict indication of operation, the combined operation of nasal septum mucosa separation or submucosal correction of nasal septum, the opening of anterior ethmoid chamber and partial resection of inferior turbinate achieved significant short-term results. 3. The operation method is simple and can be carried out by doctors with certain clinical experience without special equipment under local anesthesia, and can be widely carried out without special training of personnel. 4, patient compliance is good, do not need to follow up for a long time.
