【摘要】： 目的 探討玻璃體切割聯(lián)合超聲乳化白內(nèi)障摘除并囊袋內(nèi)植入折疊人工晶體手術(shù)治療增殖期糖尿病性視網(wǎng)膜病變對角膜內(nèi)皮細(xì)胞的影響。 方法 合適的選擇增殖期糖尿病視網(wǎng)膜病變合并白內(nèi)障的病例行玻璃體切割聯(lián)合超聲乳化白內(nèi)障摘除手術(shù)并囊袋內(nèi)植入折疊型人工晶體,分別于手術(shù)前,術(shù)后1周,術(shù)后3個月以上不同時期采集術(shù)眼角膜內(nèi)皮細(xì)胞的各項分析指標(biāo),從中分析手術(shù)對角膜內(nèi)皮細(xì)胞的影響及相關(guān)因素。并同時比較行玻璃體切割聯(lián)合超聲乳化白內(nèi)障摘除術(shù)后的角膜內(nèi)皮細(xì)胞的改變,單純玻璃體切割術(shù)后的角膜內(nèi)皮細(xì)胞的改變及單純超聲乳化白內(nèi)障手術(shù)后角膜內(nèi)皮細(xì)胞的改變,從中評價糖尿病患者角膜內(nèi)皮細(xì)胞的功能及手術(shù)安全性。 結(jié)果 從2009年1月至2010年1月間收集糖尿病患著聯(lián)合手術(shù)病例23例,糖尿病患者單純超聲乳化白內(nèi)障手術(shù)25例,單純玻璃體切割糖尿病患者29例,分別隨診3個月以上。分別比較各組不同時間角膜內(nèi)皮細(xì)胞密度的變化,同時比較各組間的差異,統(tǒng)計患者未排除玻璃體充填的病例。結(jié)果發(fā)現(xiàn)：組內(nèi)比較單純玻璃體切割手術(shù)組手術(shù)前后各期角膜內(nèi)皮細(xì)胞密度差異不明顯,超聲乳化組及聯(lián)合手術(shù)組手術(shù)前后有顯著性差異；各組間比較超聲乳化組及聯(lián)合手術(shù)組無顯著差異,而兩者與單純玻璃體切割組比較差異明顯。 結(jié)論 選擇性的采用玻璃體切割聯(lián)合超聲乳化白內(nèi)障摘除手術(shù)并植入人工晶體治療增殖期糖尿病性視網(wǎng)膜病變伴有白內(nèi)障的手術(shù)是有效的,對于角膜內(nèi)皮細(xì)胞有一定的影響,但是聯(lián)合手術(shù)與單純超聲乳化白內(nèi)障手術(shù)比較并無顯著性差異,這種手術(shù)方式是安全的。
[Abstract]:Purpose. To investigate the effect of vitrectomy combined with phacoemulsification and foldable intraocular lens implantation in the treatment of proliferative diabetic retinopathy on corneal endothelial cells. Method. Vitrectomy combined with phacoemulsification was performed in proliferative diabetic retinopathy with cataract, and foldable intraocular lens was implanted in the capsular bag before and 1 week after surgery. The analysis indexes of corneal endothelial cells were collected at different time after operation more than 3 months after operation. At the same time, the changes of corneal endothelial cells after vitrectomy and phacoemulsification were compared. The changes of corneal endothelial cells after vitrectomy and the changes of corneal endothelial cells after phacoemulsification were used to evaluate the function and safety of diabetic corneal endothelial cells. Results. From January 2009 to January 2010, 23 cases of diabetic patients underwent combined surgery, 25 cases of diabetic patients underwent phacoemulsification cataract surgery and 29 cases of simple vitrectomy diabetes mellitus. The changes of corneal endothelial cell density at different time and the difference of corneal endothelial cell density in each group were compared with each other for more than 3 months. The results showed that there was no significant difference in the density of corneal endothelial cells between the two groups before and after the operation, but there was significant difference between the phacoemulsification group and the combined operation group before and after the operation. There was no significant difference between the phacoemulsification group and the combined operation group, but there was a significant difference between the phacoemulsification group and the vitrectomy group. Conclusion. Selective vitrectomy combined with phacoemulsification and intraocular lens implantation in the treatment of proliferative diabetic retinopathy with cataract is effective and has a certain effect on corneal endothelial cells. But there was no significant difference between combined surgery and phacoemulsification alone.
