有晶體眼后房型人工晶體植入術(shù)治療高度近視的早期臨床研究

【摘要】:【目的】通過觀察有晶體眼后房型人工晶體植入術(shù)手術(shù)前后裸眼視力、低對比度視力、中央前房深度、前房角、眼壓及角膜內(nèi)皮細(xì)胞密度等,分析和評價有晶體眼后房型人工晶體植入術(shù)治療高度近視的早期臨床效果。 【方法】按照擬定的入選標(biāo)準(zhǔn)選取行有晶體眼后房型人工晶體植入術(shù)治療的高度近視患者48例90眼,術(shù)后隨訪6月。記錄術(shù)前及術(shù)后各隨訪時間點(diǎn)的裸眼視力、等效球鏡度、低對比度視力、眼壓、角膜內(nèi)皮細(xì)胞密度,用pentacam眼前節(jié)全景分析儀測量各時間點(diǎn)的中央前房深度、3點(diǎn)方位前房角、角膜曲率、Q值、角膜像差等指標(biāo)。使用SPSS19.0統(tǒng)計(jì)軟件,描述術(shù)前術(shù)后各指標(biāo)的分布特征,采用非參數(shù)檢驗(yàn)對裸眼視力進(jìn)行手術(shù)前后的差異性比較,采用配對t檢驗(yàn)對其余各指標(biāo)進(jìn)行手術(shù)前后的差異性比較。 【結(jié)果】90眼均成功植入ICL。所有觀察指標(biāo)除術(shù)前裸眼視力呈偏態(tài)分布外,其余指標(biāo)術(shù)前術(shù)后各時間點(diǎn)均呈正態(tài)分布。全部患者在術(shù)后隨訪期中,裸眼視力均達(dá)到術(shù)前最佳矯正視力,52.2%的眼數(shù)超出最佳矯正視力一行及以上;等效球鏡度維持在-1.0D~+1.0D;低對比度視力較術(shù)前矯正屈光不正后測得值有所提高,角膜Q值增大,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。角膜像差中,,總像差及低階像差較術(shù)前無統(tǒng)計(jì)學(xué)差異(P0.05),高階像差則較術(shù)前有所增加,差異有統(tǒng)計(jì)學(xué)意義(P0.05);中央前房深度、3點(diǎn)方位前房角較術(shù)前降低,角膜曲率略有減小,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。角膜厚度和眼壓較術(shù)前比較均無統(tǒng)計(jì)學(xué)差異(P0.05)。角膜內(nèi)皮細(xì)胞密度較術(shù)前減少,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。對于術(shù)后有統(tǒng)計(jì)學(xué)意義變化的指標(biāo),除低對比度視力在術(shù)后三月達(dá)穩(wěn)定水平外,其余各指標(biāo)均在術(shù)后一月時即達(dá)到穩(wěn)定狀態(tài)。91.7%的患者對ICL植入術(shù)早期效果表示滿意,全部患者術(shù)后均未出現(xiàn)嚴(yán)重并發(fā)癥。 【結(jié)論】有晶體眼后房型人工晶體植入術(shù)矯正高度近視在術(shù)后早期隨訪中顯示出良好的有效性、安全性、穩(wěn)定性及可預(yù)測性,取得了初步滿意的臨床效果。
[Abstract]:[Objective] through the observation of phakic intraocular lens implantation after operation visual acuity, low contrast visual acuity, anterior chamber depth, anterior chamber angle, intraocular pressure and corneal endothelial cell density, analysis and evaluation of the early clinical effect of phakic posterior chamber intraocular lens implantation for high myopia.
[method] in accordance with the inclusion criteria for selection of intraocular lens implantation for the treatment of crystal eyes after high myopia patients 48 cases 90 eyes, postoperative follow-up of June. Record the preoperative and postoperative follow-up time points uncorrected visual acuity, spherical equivalent, low contrast visual acuity, intraocular pressure, corneal density the endothelial cells with Pentacam anterior segment analyzer panoramic measurement of central anterior chamber depth at each time point, 3 point range of anterior chamber angle, corneal curvature, corneal aberration index Q value. The use of SPSS19.0 statistical software to describe the distribution characteristics of each index before and after surgery, using non parametric test to compare the difference before and after the operation of the uncorrected visual acuity, using the paired t test of the other indicators to compare the difference before and after operation.
[results] 90 eyes were successfully implanted ICL. all the observed indexes except the preoperative uncorrected visual acuity showed a skewed distribution, the indexes before and after surgery were all normal distribution. All patients in the postoperative follow-up, visual acuity reached preoperative best corrected visual acuity, the number of 52.2% eyes beyond the best a line of vision correction and above; spherical equivalent at -1.0D ~ +1.0D; low contrast visual acuity correction of ametropia after corneal measured value increased, Q value increased, the differences were statistically significant (P0.05). Corneal aberration, total aberration and low order aberrations than before surgery had no statistical difference (P0.05), high order aberrations compared with the preoperative increased, the difference was statistically significant (P0.05); the central anterior chamber depth, anterior chamber angle range 3 points lower than the preoperative corneal curvature decreases slightly, the differences were statistically significant (P0.05). The corneal thickness and intraocular pressure were compared with the preoperative No statistical difference (P0.05). Corneal endothelial cell density decreased, the difference was statistically significant (P0.05). The postoperative change was statistically significant indicators, in addition to low contrast visual acuity after surgery in March reached a stable level, the rest of the indicators in the postoperative January is reached to the stable state of.91.7% patients were satisfied with the early effect of ICL implantation, all patients had no serious complications.
[Conclusion] posterior chamber intraocular lens implantation for correction of high myopia has good efficacy, safety, stability and predictability in early follow-up, and has achieved initial satisfactory clinical results.
