256排寬體探測器CT智能心電門控技術(shù)在不控制心率患者冠狀動脈CTA中的應(yīng)用

【摘要】:目的探討256排寬體探測器CT智能心電門控技術(shù)對不控制心率患者CT冠狀動脈成像的圖像質(zhì)量及可診斷性。方法 200例疑似冠心病的患者接受256排寬體探測器CT冠狀動脈成像,采用智能心電門控掃描,根據(jù)掃描患者實(shí)時心率分成4組,A組(心率≤69次/分)、B組(心率為70~80次/分)、C組(心率為81~90次/分)、D組(心率≥91次/分),每組50例。由2名有心血管CT診斷經(jīng)驗的醫(yī)師以盲法對4組冠狀動脈成像的圖像質(zhì)量及可診斷性進(jìn)行評價,并比較各組的輻射劑量。結(jié)果 4組患者年齡、性別、體質(zhì)量指數(shù)差異無統(tǒng)計學(xué)意義;圖像質(zhì)量主觀評分差異有統(tǒng)計學(xué)意義(P0.05)。200例患者共評價600支血管2 575個節(jié)段,4組圖像節(jié)段水平、患者水平、血管水平的可診斷率差異均無統(tǒng)計學(xué)意義(P均0.05)。A~D組的有效輻射劑量分別為(1.05±0.48)mSv、(2.41±1.20)mSv、(1.27±0.55)mSv、(2.66±1.12)mSv,差異有統(tǒng)計學(xué)意義(F=29.22,P0.001)。結(jié)論 Revolution CT寬體探測器智能心電門控技術(shù)在不同心率患者單個心動周期完成冠狀動脈CTA檢查具有可行性,并提高了中高心率患者檢查成功率及圖像質(zhì)量。
