老年慢性心力衰竭患者可溶性致癌抑制因子2的水平及臨床意義

【摘要】:目的探討血清可溶性致癌抑制因子2(s ST2)與紐約心臟病協(xié)會(huì)(NYHA)心功能分級(jí)的相關(guān)性及對(duì)老年慢性心力衰竭的診斷價(jià)值。方法選取年齡60歲、有明確診斷為慢性心力衰竭的老年患者108例為研究組,老年健康體檢者53例作為對(duì)照組。入組后抽血測(cè)定N端腦鈉肽前體(NT-pro BNP)、s ST2、心肌肌鈣蛋白(c Tn)I2、肌紅蛋白(MYO)、肌酸激酶同工酶(CK-MB),48 h內(nèi)完成超聲心動(dòng)圖檢查。結(jié)果 s ST2在心力衰竭組較對(duì)照組明顯升高〔(31.2±15.3)vs(12.5±9.8)μg/L,P0.05〕,不同心功能級(jí)別之間s ST2濃度差異顯著(F=48.63,P0.01);研究組中s ST2和NT-pro BNP明顯正相關(guān)(r=0.631,P0.05);s ST2和左心室射血分?jǐn)?shù)(LVEF)明顯負(fù)相關(guān)(r=-0.682,P0.05)。結(jié)論慢性心力衰竭患者中s ST2明顯升高,且隨NYHA分級(jí)升高有上升趨勢(shì),是評(píng)價(jià)老年心功能較為敏感的實(shí)驗(yàn)室指標(biāo)。
