高密度脂蛋白組成部分中的不同亞類對冠狀動脈狹窄的嚴重程度影響的相關性研究

【摘要】:目的探討冠脈造影患者高密度脂蛋白(High-density lipoprotein,HDL)亞類對冠狀動脈狹窄程度影響的相關性。方法(1)、抽取2013年-2014年因疑診冠心病(Coronary Heart Disease,CHD)入住我院老年醫(yī)學科、心內科行冠狀動脈造影(Coronary arteriography,CAG)的患者122例,經(jīng)選擇性冠狀動脈造影確診為CHD患者(至少有一支冠狀動脈或分支狹窄50%)96例,其中男62例,女34例,平均年齡(60.31±8.72),未診斷CHD患者26例,男10例,女16例,平均年齡(59.00±13.51);(2)、抽取患者空腹12 h靜脈血,選擇雙向電泳-免疫印跡檢測法24h內測定HDL亞類,對患者冠脈造影結果(采用Gensini評分)評價冠狀動脈狹窄程度;(3)、計量資料用均數(shù)±標準差(?X±s)表示,定性資料用Χ2檢驗,兩組比較采用獨立樣本的t檢驗,logisitic回歸分析檢驗與冠心病相關因素,兩因素相關性采用線性相關分析。結果(1)單因素卡方、兩樣本獨立t檢驗顯示性別、吸煙史、糖尿病史、收縮壓、體質指數(shù)(Body Mass Index,BMI)、腰圍(Waist circuit,WC)、腰臀比(waist to hip ratio,WHR)、HDL-C、LDL-C、Apo A-I、Lpa、Preb1-HDL、HDL3b、HDL3c、HDL2b與冠心病有關,差異有統(tǒng)計學意義(P0.05);(2)TG、Preb1-HDL、HDL3b是冠心病危險因素,HDL2b是冠心病保護因素(P0.05);(3)HDL2b與Gensini評分結果成負相關,Preb1-HDL、HDL3b與Gensini評分結果成正相關。結論在臨床診斷治療中,HDL2b值有可能做為冠心病預測因素,Preβ1-HDL、HDL3b、HDL2b值可為預測和診斷CHD提供重要依據(jù),冠心病的防治不僅要改善和促進膽固醇逆轉運還要調節(jié)血脂,還要調整HDL亞類的構成比。。但本組研究病例數(shù)量較少,隨訪時間較短,仍需更多病例長期隨訪來綜合評價其臨床效果。
[Abstract]:Objective to investigate the effect of high density lipoprotein (High-density lipoprotein,HDL) subclasses on the degree of coronary artery stenosis in patients with coronary angiography. Methods (1) from 2013 to 2014, 122 patients with suspected coronary heart disease (Coronary Heart Disease,CHD) admitted to the department of geriatrics and underwent coronary angiography (Coronary arteriography,CAG) in our hospital. 96 patients with CHD (at least one coronary artery or branch stenosis 50%) were diagnosed by selective coronary angiography, including 62 males and 34 females with an average age of 60.31 鹵8.72. There were 26 patients with undiagnosed CHD, 10 males and 16 females. The average age was 59.00 鹵13.51). (2) the fasting venous blood of the patients was taken for 12 hours, and the HDL subclasses were measured within 24 hours by two-dimensional electrophoresis and immunoblotting. The coronary angiography results (Gensini score) were used to evaluate the degree of coronary artery stenosis. (3) the econometric data were expressed by mean 鹵standard deviation (? X 鹵s), the qualitative data were tested by X 2 test, the two groups were compared with t test of independent samples, logisitic regression analysis was used to test the related factors of coronary heart disease, and linear correlation analysis was used to analyze the correlation between the two factors.
