【摘要】：目的探討累積高血壓暴露和臂踝動(dòng)脈脈搏波傳導(dǎo)速度的關(guān)聯(lián)。方法在參與開(kāi)灤集團(tuán)第1次健康體檢(2006年~2007年)的101510名開(kāi)灤職工中,按照分層隨機(jī)抽樣標(biāo)準(zhǔn)方式抽取觀察對(duì)象,其中符合入選標(biāo)準(zhǔn)者5440人,對(duì)上述人群進(jìn)行了第4次體檢(2012年~2013年),并測(cè)量了該人群的臂踝動(dòng)脈脈搏波傳導(dǎo)速度(ba PWV),最后共有3676人納入統(tǒng)計(jì)分析。將人群按照高血壓暴露時(shí)間分為累積高血壓暴露0年、2年、4年、6年、8年共5組,采用了Logistic回歸方法分析了不同累積高血壓暴露組與ba PWV≥1400cm/s的關(guān)聯(lián)。結(jié)果在3676名觀察對(duì)象中,平均年齡為55.70±9.84歲。其中,男性2012人(54.7%),女性1664人(45.3%)。隨著高血壓累積暴露時(shí)間的延長(zhǎng),ba PWV均值和ba PWV≥1400cm/s的比例均逐漸增高(均P0.05)。在Logistic回歸分析中,校正了相關(guān)因素后,結(jié)果提示,與累積高血壓暴露0年組相比,暴露2年組、4年組、6年組、8年組均是發(fā)生ba PWV≥1400cm/s的危險(xiǎn)因素,OR值(95%CI)分別為1.76(1.38~2.25)、2.06(1.54~2.77)、3.14(2.18~4.53)、3.05(1.96~4.74);在比較回歸方程的擬合情況時(shí)發(fā)現(xiàn)累積高血壓暴露與ba PWV的關(guān)聯(lián)強(qiáng)于基線高血壓暴露。結(jié)論長(zhǎng)時(shí)間的累積高血壓暴露是ba PWV≥1400cm/s的危險(xiǎn)因素;累積高血壓暴露與ba PWV的關(guān)聯(lián)強(qiáng)于基線高血壓暴露。
[Abstract]:Objective to investigate the relationship between cumulative hypertension exposure and brachial and malleolus artery pulse wave velocity (PWV). Methods 101510 Kailuan staff and workers who participated in the first health check-up of Kailuan Group (2006-2007) were selected according to the stratified random sampling standard, of which 5440 met the selection criteria. The fourth physical examination (2012-2013) was carried out and the brachial-ankle artery pulse wave velocity (ba PWV),) of the population was measured. A total of 3676 people were included in the statistical analysis.
