無創(chuàng)血管功能檢測在早期動脈硬化患者中的應(yīng)用及相關(guān)因素分析

【摘要】:目的:通過無創(chuàng)血管功能篩檢技術(shù)在早期動脈硬化患病人群中的運用,探討無創(chuàng)血管功能檢測指標(biāo)--脈搏波傳導(dǎo)速度在衡量動脈硬化危害及動脈彈性異常程度中的應(yīng)用價值,分析與其主要影響因素之間的相關(guān)性,以便為動脈硬化的早期防治提供一些肯定的參考依據(jù)。方法:收集于皖南醫(yī)學(xué)院附屬醫(yī)院弋磯山醫(yī)院健康體檢中心在2015年1月-2016年7月期間進(jìn)行體格檢查者601例(男性358例,女性243例,年齡范圍在21歲-80歲之間)作為研究對象。采用VBP-9 120806005動脈硬化檢測儀(北京鑫悅琦科貿(mào)有限公司)檢驗臂(肱)踝脈搏波傳導(dǎo)速度(brachial ankle pulse wave velocity,baPWV)和踝臂指數(shù)(ankle brachial index,ABI),同步記錄反射波增強(qiáng)指數(shù)(augmentation index,AI)和心率(heart rate,HR),并使用該儀器內(nèi)自帶的調(diào)查卷同步采集被檢者其它有關(guān)信息。根據(jù)臂踝脈搏波傳導(dǎo)速度(brachial ankle pulse wave velocity,baPWV)1400cm/s為異常的基準(zhǔn)值[1],以左右兩側(cè)較高測量值為定組標(biāo)準(zhǔn),將受檢者界定為動脈硬化組和非動脈硬化組,其中baPWV1400cm/s為動脈硬化組,共301例,baPWV≤1400cm/s為非動脈硬化組,共300例。采集被檢者清晨進(jìn)餐前靜脈血進(jìn)行尿酸、肌酐、腎小球濾過率、胱抑素C、血糖、糖化血紅蛋白、血脂等生化標(biāo)志物的檢測,并記錄結(jié)果。采用SPSS 18.0統(tǒng)計學(xué)分析軟件進(jìn)行t檢驗、卡方檢驗及Logistic回歸分析等,以P0.05為差異有統(tǒng)計學(xué)意義。結(jié)果:1、動脈硬化組和非動脈硬化組臨床資料對比得出:baPWV與年齡、體重、BMI、SBP、DBP、MAP、HR、肌酐、尿酸、胱抑素C、BS、HBA1C、TC、TG及LDL-C存在正性關(guān)聯(lián),與L-ABI、R-ABI、HDL-C及GFR呈負(fù)性關(guān)聯(lián)。2、baPWV與各因素的Logistic回歸分析顯示:年齡、SBP、血尿酸、胱抑素C和血糖是baPWV的獨立影響因素。結(jié)論:1.年齡、SBP、血尿酸、血糖和胱抑素C是baPWV的獨立影響因素。2.年齡及收縮壓與動脈硬化的嚴(yán)重度呈正相關(guān),即隨著年齡及收縮壓的增加動脈彈性異常度越重。3.血尿酸及胱抑素C濃度與動脈硬化之間存在正性關(guān)聯(lián),其濃度高低能反應(yīng)動脈硬化病變的嚴(yán)重度。
[Abstract]:Objective: to explore the application value of pulse wave velocity (PWV), a non-invasive index of blood vessel function detection, in evaluating the damage of arteriosclerosis and the degree of arterial elasticity abnormality through the application of non-invasive vascular function screening technique in patients with early arteriosclerosis. In order to provide some definite reference basis for the early prevention and treatment of arteriosclerosis, the correlation between them and the main influencing factors were analyzed. Methods: from January 2015 to July 2016, 601 cases (358 males and 243 females) underwent physical examination in Yijishan Hospital, affiliated Hospital of Southern Anhui Medical College. The age range ranged from 21 to 80 years of age.
