腰臀比縱向軌跡與動(dòng)脈硬化的關(guān)聯(lián)性

【摘要】:目的探討腰臀比縱向軌跡與動(dòng)脈硬化的關(guān)聯(lián)。方法研究隊(duì)列由開灤卒中隊(duì)列、老年人群隊(duì)列、妊娠隊(duì)列和外周血管疾病篩查隊(duì)列組成,其中符合入選標(biāo)準(zhǔn)的研究對(duì)象共9080名。通過SAS Proc Traj程序確定觀察對(duì)象2006-2007、2008-2009、2010-2011年健康體檢的腰臀比縱向軌跡,生成4組不同的腰臀比軌跡組:低-中軌跡組、中-穩(wěn)定軌跡組、高-高軌跡組、高-中軌跡組,并于2010-2015年對(duì)觀察對(duì)象進(jìn)行了肱踝動(dòng)脈脈搏波傳導(dǎo)速度(baPWV)檢測(cè)。采用協(xié)方差分析校正了年齡、性別后不同腰臀比縱向軌跡分組baPWV的差異。采用多因素Logistic回歸模型分析腰臀比縱向軌跡與動(dòng)脈硬化的關(guān)聯(lián)。定義baPWV≥1400cm/s為動(dòng)脈硬化。結(jié)果與其他腰臀比軌跡組相比,高-高軌跡組的baPWV值[(1545.56±4.31)cm/s]、動(dòng)脈硬化人群比例(67.5%)均最高。低-中軌跡組、中-穩(wěn)定軌跡組、高-高軌跡組、高-中軌跡組動(dòng)脈硬化的發(fā)生率分別為:28.8%、50.0%、67.5%、55.4%。在多因素Logistic回歸分析中,校正了其他混雜因素后,與低-中軌跡組相比,中-穩(wěn)定軌跡組、高-高軌跡組、高-中軌跡組是動(dòng)脈硬化發(fā)生的危險(xiǎn)因素,對(duì)應(yīng)的OR值(95%CI)分別為1.42(1.15~1.76)、1.80(1.42~2.29)、1.63(1.11~2.38),均P0.05。此外,分別除外服用降壓藥、降糖藥、降脂藥人群后進(jìn)行了敏感性分析,結(jié)果與主要研究結(jié)果趨勢(shì)一致。結(jié)論腰臀比縱向軌跡水平升高是發(fā)生動(dòng)脈硬化的危險(xiǎn)因素。

Methods the study cohort consisted of Kailuan stroke cohort, elderly population cohort, pregnancy cohort and peripheral vascular disease screening cohort.
