冠心病合并糖尿病患者介入治療后血清脂質(zhì)過氧化物水平的變化

【摘要】:目的:觀察冠狀動(dòng)脈性心臟病并患有糖尿病的患者,經(jīng)橈動(dòng)脈介入(PCI)術(shù)后脂質(zhì)過氧化損傷指標(biāo)的變化及冠脈病變進(jìn)展情況。方法:選擇38例冠心病患者(A組)及42例冠心病合并糖尿病患者(B組)進(jìn)行PCI,分別在介入治療前與治療后即刻,30min,1h,24h及7d采血測(cè)定丙二醛(MDA)和超氧化物歧化酶(SOD)水平,并動(dòng)態(tài)隨訪PCI術(shù)后冠狀動(dòng)脈病變的情況。結(jié)果:1.術(shù)后兩組患者M(jìn)DA與SOD水平均較術(shù)前有明顯變化,脂質(zhì)過氧化物指標(biāo)明顯變化,但這種變化在B組更加明顯,至術(shù)后第7天,B組仍有明顯變化,而A組已經(jīng)接近術(shù)前水平。2.隨訪12個(gè)月發(fā)現(xiàn),B組患者再狹窄,死亡,心血管事件發(fā)生顯著高于A組(P0.05),卒中發(fā)生率無明顯差異(P0.05),χ2值分別為4.20、4.21、5.17、0.30、22.33。結(jié)論:冠心病合并糖尿病患者PCI術(shù)后出現(xiàn)脂質(zhì)過氧化損傷指標(biāo)的變化持續(xù)且時(shí)間長(zhǎng),糖尿病可能促進(jìn)了這一損傷過程。
[Abstract]:Aim: to observe the changes of lipid peroxidation injury and the progression of coronary artery disease in patients with coronary heart disease (CHD) and diabetes mellitus (DM) after transradial intervention (PCI). Methods: PCI, was performed in 38 patients with coronary heart disease (group A) and 42 patients with coronary heart disease complicated with diabetes (group B) before and immediately after interventional therapy. The levels of malondialdehyde (MDA) and superoxide dismutase (SOD) were measured at 24 h and 7 d, and the coronary artery lesions after PCI were followed up dynamically. Results: 1. The levels of MDA and SOD in the two groups were significantly different from those before operation, but the changes of lipid peroxides were more obvious in group B than those before operation. At the 7th day after operation, the levels of MDA and SOD in group B were still significantly changed, but the level of lipid peroxide in group A was close to the preoperative level. 12 months follow-up showed that restenosis, death and cardiovascular events in group B were significantly higher than those in group A (P0.05), and there was no significant difference in the incidence of stroke (P0.05). Conclusion: the changes of lipid peroxidation in patients with coronary heart disease complicated with diabetes mellitus after PCI are continuous and long. Diabetes may promote the process of this injury.
