【摘要】：目的探討溫陽利水活血湯聯(lián)合西藥治療慢性收縮性心力衰竭陽虛血瘀證的臨床療效。 方法選取確診為慢性收縮性心衰,且辨證為陽虛血瘀證的患者60例,按入院先后隨機分為治療組及對照組各30例。兩組均采用無差別的西醫(yī)治療,治療組另加溫陽利水活血湯治療。觀察治療前和治療14天后患者的中醫(yī)證候積分改善情況、心功能分級、NT-proBNP(N末端腦鈉肽前體)、LVEF(左室射血分數(shù))變化。 結(jié)果(1)治療后兩組在中醫(yī)證候積分改善方面,治療組總有效率為86.66%,對照組總有效率為76.66%,兩組療效對比,P0.05,治療組療效優(yōu)于對照組。 (2)兩組血漿NT--proBNP及LVEF治療前后對比均明顯改善,P0.05,治療后兩組血漿NT--proBNP及LVEF對比p0.05,治療組療效優(yōu)于對照組。 (3)治療組心功能分級改善為93.33%,對照組為70.00%,兩組對比,P0.05,治療組優(yōu)于對照組。 結(jié)論溫陽利水活血湯聯(lián)合西藥治療辨證為陽虛血瘀證的慢性心力衰竭有較好的臨床療效,且安全性較高。
[Abstract]:Objective to investigate the clinical effect of Wenyang Lishui Huoxue decoction combined with western medicine in the treatment of chronic systolic heart failure with yang deficiency and blood stasis syndrome. Methods Sixty patients with chronic systolic heart failure and syndrome differentiation of yang deficiency and blood stasis were randomly divided into treatment group (n = 30) and control group (n = 30). Both groups were treated by western medicine without difference, and the treatment group was treated with warming Yang Li Shui Huo Xue Tang. The improvement of TCM syndromes score, cardiac function grading and NT-proBNP (left ventricular ejection fraction) of N-terminal brain natriuretic peptide precursor (), LVEF () were observed before and 14 days after treatment. Results (1) after treatment, the total effective rate was 86.66 in the treatment group and 76.66 in the control group. The curative effect of the treatment group was better than that of the control group (P 0.05). (2) the plasma NT--proBNP and LVEF of the two groups were significantly improved before and after treatment, P 0.05. After treatment, the plasma NT--proBNP and LVEF of the two groups were compared with that of the control group. The curative effect of the treatment group was better than that of the control group. (3) the improvement of cardiac function grade in the treatment group was 93.33 and that in the control group was 70.00.The difference between the two groups was P0.05. the treatment group was superior to the control group. Conclusion Wenyang Lishui Huoxue decoction combined with western medicine in treating chronic heart failure with syndrome differentiation of yang deficiency and blood stasis has better clinical effect and higher safety.
