冠心病患者血清糖化血紅蛋白A1c型水平與冠狀動(dòng)脈狹窄程度的相關(guān)性研究

【摘要】:目的:探討冠心病(CHD)患者血清糖化血紅蛋白A1c型(GHbA1c)水平與冠狀動(dòng)脈狹窄程度的關(guān)系及其作用機(jī)理。方法:選取126例經(jīng)冠狀動(dòng)脈造影(CAG)檢查確診為冠心病(CHD)的患者分成2組,其中,冠心病組67例,冠心病并糖尿病(DM)組59例。與此同時(shí),選取除外CHD和DM的人群46例作為對(duì)照組。全部研究對(duì)象測(cè)定血清糖化血紅蛋白A1c型(GHbA1c)水平及一般生化指標(biāo),并以Gensini積分值作為評(píng)估冠狀動(dòng)脈狹窄程度的指標(biāo)。分析CHD患者血清GHbA1c水平與冠狀動(dòng)脈狹窄程度之間的相關(guān)性。結(jié)果:CHD組的血清GHbA1c水平(6.16±0.43%)及Gensini積分值(74.5±16.5)均高于對(duì)照組(5.52±0.41%,32.5±15.5)(P㩳0.01),CHD合并DM組的血清GHbA1c水平(7.38±0.39%)及Gensini積分值(80.5±13.5)均顯著高于CHD組和對(duì)照組(P㩳0.01)。采用Spearman秩相關(guān)及多因素多元線性回歸分析顯示,血清GHbA1c水平與Gensini積分值呈正相關(guān)(=0.367,P㩳0.01)。結(jié)論:CHD患者血清GHbA1c水平與冠狀動(dòng)脈狹窄程度呈正相關(guān)。血清GHbA1c水平是影響CHD冠狀動(dòng)脈功能的重要危險(xiǎn)因子。
[Abstract]:Objective: to investigate the relationship between serum glycosylated hemoglobin A 1c (GHbA1c) level and coronary artery stenosis in patients with coronary heart disease (CHD). Methods: 126 patients with coronary heart disease (CHD) diagnosed by coronary angiography (CAG) were divided into two groups: coronary heart disease (CHD) group (n = 67), coronary heart disease (CHD) group (n = 59) and diabetes mellitus (DM) group (n = 59). At the same time, 46 subjects excluding CHD and DM were selected as control group. Serum glycosylated hemoglobin A1c (GHbA1c) levels and general biochemical indexes were measured, and the Gensini score was used to evaluate the severity of coronary artery stenosis. To analyze the correlation between serum GHbA1c level and severity of coronary artery stenosis in patients with CHD. Results: the levels of serum GHbA1c (6.16 鹵0.43%) and Gensini integral (74.5 鹵16.5) in CHD group were significantly higher than those in control group (5.52 鹵0.41 鹵32.5 鹵15.5) (P < 0.01), GHbA1c level (7.38 鹵0.39%) and Gensini score (80.5 鹵13.5) in DM group were significantly higher than those in CHD group and control group (P0. 01). The Spearman rank correlation and multivariate linear regression analysis showed that the serum GHbA1c level was positively correlated with the Gensini score (= 0.367 P0. 01). Conclusion: the level of serum GHbA1c in patients with CHD is positively correlated with the degree of coronary artery stenosis. Serum GHbA1c level is an important risk factor affecting coronary artery function of CHD.
