【摘要】：目的:探討microRNA-182(miR-182)、cAMP應(yīng)答元件結(jié)合蛋白反式作用因子(CITED2)及低氧誘導(dǎo)因子-1(HIF-1)在缺血性心肌病(ICM)患者外周血中的表達(dá)及相關(guān)性。方法:收集本院46例ICM患者設(shè)為ICM組,同期選擇32例健康者為對(duì)照組。分析2組一般資料及心臟功能指標(biāo)[氨基末端B型腦鈉肽前體(NTproBNP)、左室射血分?jǐn)?shù)(LVEF)、6min步行實(shí)驗(yàn)(6MWT)距離];用實(shí)時(shí)熒光定量聚合酶鏈?zhǔn)椒磻?yīng)法(qRT-PCR)檢測(cè)血漿miR-182的含量,采用ELISA法測(cè)定血清CITED2和HIF-1水平,并觀察ICM組治療前及治療3個(gè)月后上述指標(biāo)的變化;分析ICM患者血漿miR-182水平與有關(guān)因素的相關(guān)性。結(jié)果:ICM組治療前CITED2、LVEF及6MWT距離明顯低于對(duì)照組,NT-proBNP、HIF-1及miR-182表達(dá)明顯高于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。與ICM組治療前比較,ICM組治療后CITED2、LVEF及6MWT距離明顯升高,NT-proBNP、HIF-1及miR-182顯著降低,差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。ICM組治療前血漿miR-182的水平與CITED2、LVEF及6MWT距離成負(fù)相關(guān)(r=-0.427、-0.351、-0.298,P0.05);ICM組治療前血漿miR-182水平與HIF-1成正相關(guān)(r=0.337,P0.05)。結(jié)論:外周血miR-182水平可作為ICM診斷及進(jìn)行風(fēng)險(xiǎn)評(píng)估的生物標(biāo)志物。miR-182可能通過其靶蛋白CITED2上調(diào)HIF-1,參與調(diào)控ICM的發(fā)生及發(fā)展。
[Abstract]:Aim: to investigate the expression and correlation of microRNA-182 (miR-182) camp response element binding protein trans action factor (CITED2) and hypoxia inducible factor 1 (HIF-1) in peripheral blood of (ICM) patients with ischemic cardiomyopathy. Methods: 46 patients with ICM in our hospital were divided into ICM group and 32 healthy persons as control group. The general data and cardiac function index of the two groups were analyzed [left ventricular ejection fraction (6MWT) distance of left ventricular ejection fraction (LVEF) of amino-terminal B-type brain natriuretic peptide precursor (NTproBNP),)], real-time fluorescence quantitative polymerase chain reaction (qRT-PCR) was used to detect the content of plasma miR-182. The levels of serum CITED2 and HIF-1 were measured by ELISA method, and the changes of these indexes before and after 3 months of treatment in ICM group were observed, and the correlation between plasma miR-182 level and related factors in ICM patients was analyzed. Results the distance between CITED2,LVEF and 6MWT in the control group was significantly lower than that in the control group (P0.01). The expression of HIF-1 and miR-182 in the control group was significantly higher than that in the control group (P0.01). The distance between CITED2,LVEF and 6MWT in ICM group was significantly higher than that in ICM group (P 0.01). The plasma miR-182 level in ICM group was negatively correlated with CITED2,LVEF and 6MWT distance (r-0.427) -0.351 ~ (-1) -0.298 (P0.05). There was a positive correlation between plasma miR-182 level and HIF-1 in ICM-treated group (r = 0.337). Conclusion: the level of miR-182 in peripheral blood can be used as a biomarker for the diagnosis and risk assessment of ICM. MiR-182 may regulate the occurrence and development of ICM by upregulating HIF-1, through its target protein CITED2.
