【摘要】：目的探討高血壓患者合并冠心病(CAD)的危險因素及相關(guān)性。方法納入2015年8月至2016年12月安徽醫(yī)科大學(xué)第一附屬醫(yī)院心內(nèi)科收治的高血壓病患者258例,根據(jù)冠狀動脈造影結(jié)果分為高血壓組(124例),高血壓合并冠心病組(134例)。記錄研究對象的一般資料、血小板參數(shù)、血脂水平、腎功能指標。比較兩組患者各指標的差異,并分析這些指標與高血壓患者并發(fā)冠心病的相關(guān)關(guān)系。結(jié)果(1)與高血壓組患者相比,高血壓合并冠心病組患者的年齡較大、吸煙者較多、血小板分布寬度、平均血小板體積(MPV)、肌酐、血尿酸的值較高,而HDL-C水平較低,差異均有統(tǒng)計學(xué)意義(P0.05)。(2)多因素Logistic回歸分析顯示:年齡、吸煙史、MPV、血尿酸及低水平的HDL-C是高血壓患者合并冠心病的獨立危險因素[OR=1.062,95%CI:1.032~1.093;OR=2.048,95%CI:1.078~3.893;OR=2.737,95%CI:1.193~6.278;OR=1.006,95%CI:1.001~1.010;OR=0.280,95%CI:0.115~0.681(均P0.05)]。(3)Pearson相關(guān)分析結(jié)果顯示:MPV和血尿酸呈正相關(guān)(r=0.17,P0.05)。結(jié)論年齡、吸煙史、MPV、血尿酸和較低的HDL-C值是高血壓患者并發(fā)冠心病的危險因素,應(yīng)當積極檢測并預(yù)防,以降低高血壓合并冠心病的發(fā)病風(fēng)險。
[Abstract]:Objective to investigate the risk factors and correlation of coronary heart disease (CAD) in patients with hypertension. Methods from August 2015 to December 2016, 258 patients with hypertension were enrolled in the Department of Cardiology, first affiliated Hospital of Anhui Medical University. According to the results of coronary angiography, 258 patients were divided into hypertension group (124 cases) and hypertension with coronary heart disease group (134 cases). The general data, platelet parameters, blood lipid level and renal function were recorded. To compare the differences between the two groups and to analyze the correlation between these indexes and coronary heart disease (CHD) in patients with hypertension.
