【摘要】：目的分析家庭醫(yī)生簽約高血壓患者健康管理服務(wù)的成本和效果。方法選取2012—2014年上海市天目西路社區(qū)衛(wèi)生服務(wù)中心家庭醫(yī)生簽約高血壓患者及公共衛(wèi)生人員管理高血壓患者,調(diào)查家庭醫(yī)生次均門診醫(yī)療服務(wù)人力成本、家庭醫(yī)生門診高血壓健康管理服務(wù)人力成本、公共衛(wèi)生人員服務(wù)人力總成本、家庭醫(yī)生與公共衛(wèi)生人員高血壓健康管理模式成本-效果分析。結(jié)果 2012—2014年家庭醫(yī)生高血壓接受規(guī)范管理比例及血壓控制達(dá)標(biāo)率均高于公共衛(wèi)生人員。2012—2014年家庭醫(yī)生門診高血壓健康管理平均每有效控制血壓一人的平均成本分別為1.06、1.59、1.53元;公共衛(wèi)生人員分別為28.19、35.34、44.57元;每有效控制血壓一人的平均成本前者比后者分別節(jié)省27.13、33.75、43.04元。結(jié)論家庭醫(yī)生簽約高血壓患者實(shí)施健康管理是一種低成本高效果的慢病管理模式。
[Abstract]:Objective to analyze the cost and effect of health management services for hypertensive patients contracted by family doctors. Methods Family doctors of community health service center of Tianmu Xilu in Shanghai from 2012 to 2014 were selected to contract hypertension patients and public health workers to manage hypertension patients. The cost of hypertension health management service in outpatient clinic of family doctors, the total cost of public health personnel, and the cost-effect analysis of family doctor and public health personnel hypertension health management model. Results in 2012-2014, the proportion of family doctors accepting standard management of hypertension and the rate of meeting the standard of blood pressure control were higher than those of public health workers. The average health management of hypertension in outpatient clinics of family doctors in 2012-2014 was higher than that of one person who effectively controlled blood pressure. The cost was 1.06%, 1.59% and 1.53 yuan respectively. The average cost of each person with effective blood pressure control was 27.1333.75,43.04 yuan less than that of the latter. Conclusion the health management of hypertensive patients contracted by family doctors is a low-cost and high-effect chronic disease management model.
