【摘要】：目的:觀察中藥復方結合增強型體外反搏(enhanced external counter pulsation,EECP)治療冠心病不穩(wěn)定型心絞痛(unstable angina,UA)痰瘀互結證的臨床療效及對腫瘤壞死因子-α(tumor necrosis factor-α,TNF-α)、單核細胞趨化蛋白-1(Monocyte chemoattractant protein-1,MCP-1)的影響,并對其機制進行探索。方法:隨機選取自2014年10月至2016年2月,于遼寧省中醫(yī)院慢病六科就診,并符合冠心病不穩(wěn)定型心絞痛診斷標準的120例患者,按照隨機對照原則將受試者分為治療組和對照組各60例。對照組予常規(guī)西藥加用中藥復方治療,觀察4周。治療組在常規(guī)西藥及中藥復方治療基礎上聯(lián)合體外反搏治療,每天一次,每次60分鐘,觀察4周。4周后對比治療前后患者血清TNF-α及MCP-1水平變化情況,同時記錄兩個組別治療前后心絞痛發(fā)作及中醫(yī)癥狀的變化情況。結果:1.心絞痛癥狀療效:治療組平均秩值大于對照組,兩組間心絞痛癥狀療效具有顯著差異(Z=-3.530,P0.05),治療組優(yōu)于對照組。2.中醫(yī)證候療效:治療組平均秩值大于對照組,存在明顯差異(Z=-3.806,P0.05),治療組中醫(yī)證候療效優(yōu)于對照組。3.血清TNF-α水平:兩組治療前對比,血清TNF-α含量無統(tǒng)計學差異(P=0.969);兩組治療后血清TNF-α含量顯著降低,血清TNF-α含量存在統(tǒng)計學差異(P0.05);兩組治療后血清TNF-α水平均較治療前降低,差異有統(tǒng)計學意義(P0.05)。治療組優(yōu)于對照組.4.血清MCP-1水平:兩組治療前比較,血清MCP-1含量相比無統(tǒng)計學差異(P=0.471);兩組治療后血清MCP-1含量均降低,血清MCP-1含量有統(tǒng)計學差異(P0.05);兩組治療后血清MCP-1水平較治療前降低,差異存在統(tǒng)計學意義(P0.05)。治療組優(yōu)于對照組.結論:1.中藥復方聯(lián)合體外反搏能顯著改善冠心病不穩(wěn)定型心絞痛患者心絞痛癥狀及中醫(yī)證候。2.中藥復方與體外反搏聯(lián)合可能影響冠心病患者血清腫瘤壞死因子-α(TNF-α)水平及血清單核細胞趨化蛋白-1(MCP-1)水平。3.中藥復方聯(lián)合體外反搏治療冠心病不穩(wěn)定性心絞痛療效確切,不良反應較少,患者耐受性良好,值得推廣應用于冠心病不穩(wěn)定型心絞痛(痰瘀互結證)。
[Abstract]:Objective: To observe the clinical efficacy of traditional Chinese medicine combined with enhanced external counter pulsation (EECP) in the treatment of unstable angina pectoris (unstable angina, UA) syndrome of phlegm and stasis syndrome of coronary heart disease (unstable angina, UA) and the tumor necrosis factor - alpha (tumor necrosis factor- alpha, TNF- alpha), and monocyte chemoattractant protein. The effect of n-1, MCP-1) and its mechanism were explored. Methods: randomly selected 120 patients from October 2014 to February 2016 in six families of slow disease in Liaoning traditional Chinese medicine hospital, and conformed to the diagnostic criteria of unstable angina pectoris. The subjects were divided into the treatment group and the control group in 60 cases according to the random control principle. The control group was given the routine West. The treatment group was treated with traditional Chinese medicine compound for 4 weeks. The treatment group was treated with external counterpulsation on the basis of conventional western medicine and traditional Chinese medicine compound treatment. The changes of serum TNF- alpha and MCP-1 levels in patients before and after 4 weeks of contrast treatment were observed after 4 weeks of.4 weeks, and the changes of angina pectoris and symptoms of traditional Chinese medicine before and after treatment were recorded. Results: 1. the curative effect of angina pectoris: the average rank value of the treatment group was greater than the control group. The curative effect of the two groups was significantly different (Z=-3.530, P0.05). The treatment group was better than the control group, the average rank of the treatment group was greater than the control group, and there was significant difference (Z=-3.806, P0.05), the therapeutic effect of the treatment group was better than that of the control group. The level of serum TNF- alpha in group.3.: there was no statistical difference in serum TNF- alpha in the two groups before treatment (P=0.969); the content of serum TNF- alpha in the two groups was significantly lower, and the serum TNF- alpha content was statistically different (P0.05); the level of serum TNF- a was lower in the two groups than before the treatment (P0.05). The treatment group was superior to the control group. Group.4. serum MCP-1 level: compared with the two groups before treatment, serum MCP-1 content was not statistically significant (P=0.471); the level of serum MCP-1 in the two groups was reduced and serum MCP-1 content was statistically different (P0.05); the level of serum MCP-1 in the two groups was lower than before treatment (P0.05). The treatment group was better than the control group. 1. the combination of traditional Chinese medicine and extracorporeal counterpulsation can significantly improve the symptoms of angina pectoris in patients with unstable angina pectoris and TCM syndrome.2. combination of traditional Chinese medicine and extracorporeal counterpulsation that may affect serum tumor necrosis factor - alpha (TNF- -) level and serum monocyte chemoattractant egg white -1 (MCP-1) level.3. combined with extracorporeal counterpulsation The treatment of unstable angina pectoris with coronary heart disease is effective, less adverse reaction and well tolerated.
