射血分?jǐn)?shù)保留與射血分?jǐn)?shù)降低心衰的生物學(xué)標(biāo)記物的對比研究

【摘要】：目的:觀察可溶性ST2(sST2)、胱抑素C(Cys C)、C反應(yīng)蛋白(CRP)、心臟肌鈣蛋白T、I(cTnT、I)及B型腦鈉肽(BNP)在射血分?jǐn)?shù)保留與射血分?jǐn)?shù)降低心衰患者中的表達(dá)水平,探討其與左室射血分?jǐn)?shù)的相關(guān)性。方法:選取在2015年12月至2017年2月在新疆自治區(qū)人民醫(yī)院醫(yī)院心內(nèi)科以心衰為主要原因住院的且大于18歲的患者86例。心衰診斷標(biāo)準(zhǔn)參照2014中國心力衰竭診斷和治療指南,由2名心內(nèi)科醫(yī)師確診,依據(jù)LVEF分為LVEF降低的心衰和LVEF保留的心衰,同期住院心功能正常者40例作為正常對照組。測定實(shí)驗(yàn)所需心衰標(biāo)志物指標(biāo),并收集一般臨床特征資料,對所得結(jié)果進(jìn)行分析。結(jié)果:三組人群一般臨床特征資料比較:年齡、性別、BMI、DBP在三組中差異無統(tǒng)計(jì)學(xué)意義;HF-PEF組和HF-REF組比較,HF-REF組的sST2、cTnT、cTnI、Cys C及BNP的表達(dá)水平較高,差異有統(tǒng)計(jì)學(xué)意義,hs-CRP在HF組之間差異無統(tǒng)計(jì)學(xué)意義;比較心衰標(biāo)記物與LVEF的相關(guān)性,BNP、sST2、cTnT、cTnI、Hs-CRP、CysC與LVEF均有相關(guān)性,相關(guān)性具有統(tǒng)計(jì)學(xué)意義。結(jié)論:1.sST2、cTnT、cTnI、Cys C及BNP等可作為區(qū)分HF-PEF和HF-REF的標(biāo)志物,有重要參考價(jià)值。不同心衰標(biāo)記物表達(dá)水平的差異提示HF-PEF和HF-REF在病理生理機(jī)制中可能存在不同。2.BNP、sST2、cTnT與LVEF表達(dá)水平與左室射血分?jǐn)?shù)相關(guān)關(guān)系較密切。
[Abstract]:Aim: to investigate the expression levels of soluble ST2 (sST2), cystatin C (Cys C) C-reactive protein (CRP), (CRP),) and type B brain natriuretic peptide (BNP) (BNP) in patients with heart failure (HF), and to explore the relationship between them and left ventricular ejection fraction (LVEF). Methods: from December 2015 to February 2017, 86 patients over 18 years old who were hospitalized in the Department of Cardiology of Xinjiang Autonomous region people's Hospital for the main cause of heart failure were selected. According to 2014 guidelines for the diagnosis and treatment of heart failure in China, the diagnostic criteria for heart failure were confirmed by two cardiologists. According to LVEF, the patients were divided into two groups: LVEF decreased heart failure and LVEF reserved heart failure. 40 patients with normal cardiac function in the same period were taken as normal control group. The necessary markers of heart failure were measured and the general clinical data were collected to analyze the results. Results: there was no significant difference in age, sex and BMI-DBP between the three groups. The expression of sST2cTnTnTnTnTnTnTnCys C and BNP in HF-REF group was higher than that in HF-REF group. There was no significant difference in hs-CRP between HF group and the correlation between BNPs ST2TnTnTnTnTnTnTnTnHs-CRPnCysC and LVEF. ConclusionsST2, cTnTnTnTnTnTnCys C and BNP can be used as markers to distinguish HF-PEF from HF-REF, which has important reference value. The differences of expression levels of markers in different heart failure suggest that HF-PEF and HF-REF may have different pathophysiological mechanisms. 2. The expression of BNPs ST2cTnT and LVEF is closely related to left ventricular ejection fraction (LVEF).
