【摘要】：目的探討血漿基質(zhì)金屬蛋白酶9(MMP-9)和核轉(zhuǎn)錄因子kappa B(NF-κB)在阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)合并高血壓發(fā)病中的作用。方法選取2014年1月至6月在該院呼吸內(nèi)科及神經(jīng)內(nèi)科行多導(dǎo)睡眠監(jiān)測的受試者,根據(jù)睡眠監(jiān)測檢查結(jié)果及是否合并高血壓進(jìn)行分組,鼾癥組23例,OSAHS組32例,OSAHS合并高血壓組31例;并根據(jù)高血壓級別對OSAHS合并高血壓組進(jìn)行亞組分型,其中OSAHS合并高血壓1級組9例,2級組9例,3級組13例。分別采用酶聯(lián)免疫吸附試驗(ELISA)和實時熒光定量PCR的方法檢測血漿MMP-9水平及單個核細(xì)胞中MMP-9和NF-κB基因的表達(dá)水平。結(jié)果OSAHS合并高血壓組血漿MMP-9水平、單個核細(xì)胞MMP-9mRNA和NF-κB mRNA的表達(dá)水平均明顯高于鼾癥組和OSAHS組,差異有統(tǒng)計學(xué)意義(P0.05)。OSAHS合并高血壓3級組血漿MMP-9水平、單個核細(xì)胞MMP-9 mRNA和NF-κB mRNA的表達(dá)水平均明顯高于OSAHS合并高血壓1級組及2級組,比較差異有統(tǒng)計學(xué)意義(P0.05)。血漿MMP-9水平、MMP-9mRNA及NF-κB mRNA的表達(dá)與呼吸暫停低通氣指數(shù)(AHI)、氧減指數(shù)(ODI)呈正相關(guān)(P0.05),與最低血氧飽和度(LSaO2)呈負(fù)相關(guān)(P0.05)。血漿MMP-9水平與單個核細(xì)胞MMP-9mRNA的表達(dá)呈正相關(guān)(P0.05)。單個核細(xì)胞MMP-9mRNA的表達(dá)與NF-κB mRNA的表達(dá)呈正相關(guān)(P0.05)。結(jié)論血漿MMP-9水平、MMP-9mRNA及NF-κB mRNA的表達(dá)水平在OSAHS合并高血壓患者中明顯增高并與疾病的嚴(yán)重程度相關(guān),MMP-9在OSAHS合并高血壓發(fā)病中起著重要作用,可能受NF-κB通路的調(diào)節(jié)。
[Abstract]:Objective to investigate the role of matrix metalloproteinase 9 (MMP-9) and nuclear transcription factor kappa B (NF- 魏 B in the pathogenesis of obstructive sleep apnea hypopnea syndrome (OSAHS) with hypertension. Methods the subjects who received polysomnography from January to June 2014 in the department of respiratory and neurology in our hospital were divided into two groups according to the results of sleep monitoring and whether they were complicated with hypertension. There were 32 OSAHS patients in the snoring group and 31 patients in the hypertension group. According to the grade of hypertension, the subgroup classification of OSAHS combined with hypertension group was carried out, including 9 cases of OSAHS with hypertension grade 1 group, 9 cases of grade 2 hypertension group, 9 cases of grade 3 group. The levels of plasma MMP-9 and the expression of MMP-9 and NF- 魏 B genes in mononuclear cells were detected by enzyme linked immunosorbent assay (ELISA) and real-time fluorescence quantitative PCR. Results the levels of plasma MMP-9, mononuclear cell MMP-9mRNA and NF- 魏 B mRNA in OSAHS with hypertension group were significantly higher than those in snoring group and OSAHS group (P0.05). The expression levels of MMP-9 mRNA and NF- 魏 B mRNA in mononuclear cells were significantly higher than those in OSAHS with hypertension grade 1 and grade 2 groups (P0.05). The expression of MMP-9 mRNA and NF- 魏 B mRNA in plasma MMP-9 was positively correlated with apnea hypopnea index (AHI),) oxygen reduction index (ODI) (P0.05), and negatively correlated with minimum oxygen saturation (LSaO2) (P0.05). Plasma MMP-9 level was positively correlated with the expression of MMP-9mRNA in mononuclear cells (P0.05). The expression of MMP-9mRNA in mononuclear cells was positively correlated with the expression of NF- 魏 B mRNA (P0.05). Conclusion the expression of MMP-9 mRNA and NF- 魏 B mRNA in plasma MMP-9 level is significantly higher in OSAHS patients with hypertension and may play an important role in the pathogenesis of OSAHS complicated with hypertension, and may be regulated by NF- 魏 B pathway.
