【摘要】：目的：血清脂聯(lián)素水平與血脂異常、糖尿病、腦血管疾病等疾病密切相關(guān),但關(guān)于脂聯(lián)素是否與冠心病相關(guān)仍存爭議。鑒于國內(nèi)對于脂聯(lián)素與心血管疾病關(guān)系的研究頗多,且研究結(jié)果未達成一致,因此,本次研究對中國人群血清脂聯(lián)素水平與冠心病的關(guān)系進行薈萃分析。方法：通過檢索國內(nèi)外主要的數(shù)據(jù)庫,按照納入與排除標(biāo)準(zhǔn)獲取已公開發(fā)表的關(guān)于中國人群血清脂聯(lián)素水平與冠心病關(guān)系的文獻,提取文獻中相關(guān)數(shù)據(jù),使用STATA 12.0統(tǒng)計學(xué)軟件進meta分析。結(jié)果：本次薈萃分析共納入27篇文獻,總樣本量為3763例,其中冠心病組為2359例,對照組為1404例。由于各研究間的存在異質(zhì)性(I2=96.5%),因此通過隨機效應(yīng)模型進行Meta分析,結(jié)果顯示中國人群的血清脂聯(lián)素水平低于對照組,差異具有統(tǒng)計學(xué)意義[SMD=-1.88,95%CI(-2.31,-1.45), P0.05]。敏感性分析提示各項研究對總體合并效應(yīng)量無明顯影響,本次Meta分析結(jié)果具有較好穩(wěn)健性。通過亞組分析發(fā)現(xiàn)在總樣本量大小、冠心病組病人數(shù)、男性比例分組、NOS評分的分組時異質(zhì)性有所下降,且分組后合并的結(jié)果穩(wěn)定。Begg秩檢驗和Egger線性回歸法均提示所納入的文獻有潛在的發(fā)表偏倚。結(jié)論：中國人群冠心病患者血清脂聯(lián)素水平明顯低于正常對照組,低血清脂聯(lián)素水平與冠心病發(fā)病風(fēng)險相關(guān)。由于所納入的文獻存在異質(zhì)性和發(fā)表偏倚,因此中國人群脂聯(lián)素與冠心病的關(guān)系仍需要進一步更多大型的、前瞻性的高質(zhì)量研究進一步探討。
[Abstract]:Objective: serum adiponectin levels are closely related to dyslipidemia, diabetes, cerebrovascular diseases and other diseases, but whether adiponectin is related to coronary heart disease is still controversial. There are many studies on the relationship between adiponectin and cardiovascular disease in China, and the results are not agreed. Therefore, the relationship between serum adiponectin level and coronary heart disease in Chinese population is analyzed in this study. Methods: the published literatures on the relationship between serum adiponectin levels and coronary heart disease in Chinese population were obtained according to the inclusion and exclusion criteria by searching the main databases at home and abroad. STATA 12.0 statistical software was used for meta analysis. Results: a total of 27 articles were included in this meta-analysis. The total sample size was 3763 cases, including 2359 cases in coronary heart disease group and 1404 cases in control group. Because of the heterogeneity among the studies (I _ 2 / 96.5%), Meta-analysis was carried out by random effect model. The results showed that the serum adiponectin level in Chinese population was lower than that in the control group, and the difference was statistically significant [SMD-1.8895 CI (-2.31 ~ 1.45), P0.05]. Sensitivity analysis showed that all studies had no significant effect on the total combined effect, and the results of this meta-analysis had good robustness. By subgroup analysis, we found that the heterogeneity of the total sample size, the number of patients in coronary heart disease group, and the proportion of males grouped by NOS score decreased. After grouping, the results were stable. Begg rank test and Egger linear regression indicated that the literature included had potential publication bias. Conclusion: the serum adiponectin level in Chinese patients with coronary heart disease is significantly lower than that in normal controls, and low serum adiponectin level is associated with the risk of coronary heart disease. Due to the heterogeneity and publication bias of the literature included, the relationship between adiponectin and coronary heart disease in Chinese population needs to be further explored.
