【摘要】：目的:本研究以老年慢性心力衰竭患者為研究對象,監(jiān)測其CA125、尿酸、總膽紅素的水平,分析探討CA125、尿酸、總膽紅素對老年慢性心力衰是否具有診斷價值,及其與心功能分級的相關性,以期提供更多的客觀依據(jù)供臨床醫(yī)師參考,對老年慢性心力衰竭的診療提供新的指導意義。方法:1.收集2015年06月至2016年06月于陜西省人民醫(yī)院心內(nèi)三科住院的符合條件的慢性心力衰竭患者臨床資料,納入符合條件的慢性心力衰竭患者供98例,以同期健康體檢者52例作為對照組,按照Framingham心力衰竭診斷標準診斷的心衰組,將選取指標進行分析與對照組之間進行單因素及多因素分析,進一步分析有統(tǒng)計學意義的指標是否有診斷價值;CA125與BNP之間進行相關性分析。2.根據(jù)美國紐約心臟協(xié)會(NYHA)標準分級,將心衰組分為三組(1組:心功能I/II級、2組:心功能III級,3組.心功能IV級),分析所選取指標與心功能分級等級的相關性。所有數(shù)據(jù)均采用SPSS19.0軟件進行統(tǒng)計分析,以P0.05為差異具有統(tǒng)計學上的顯著性意義。結果:1.心衰組與對照組比較:BNP值對是否患病進行診斷的準確度高達83.5%,CA125對是否患病進行診斷的準確度高達73.8%。2.CA125與BNP的相關性分析:BNP和CA125之間存在著顯著的正向相關關系,二者之間的相關系數(shù)為0.5660。3.各項指標與心功能分級的等級相關性分析:總膽紅素與心功能分級相關系數(shù)為0.836,尿酸與心功能分級相關系數(shù)為0.463,BNP與心功能分級相關系數(shù)為0.488,CA125心功能分級相關系數(shù)為0.679。結論:1.BNP及CA125對分別對慢性心力衰竭具有診斷價值。2.慢性心力衰竭患者外周血清CA125、血尿酸、總膽紅素含量隨NYHA心功能分級增高,對于慢性心力衰竭患者癥狀的嚴重程度具有良好評價功能。
[Abstract]:Objective: to investigate the diagnostic value of CA125, uric acid and total bilirubin in elderly patients with chronic heart failure. In order to provide more objective basis for clinicians, and to provide new guidance for the diagnosis and treatment of chronic heart failure in the elderly. Method 1: 1. From June 2015 to June 2016, clinical data of eligible patients with chronic heart failure (CHF) were collected and 98 eligible CHF patients were included. In the same period, 52 healthy persons were used as the control group. The selected indexes were analyzed by single factor analysis and multivariate analysis between the two groups according to the diagnosis criteria of Fraingham heart failure. Further analysis of the statistical significance of whether there is diagnostic value of CA125 and BNP correlation analysis. 2. 2. According to the New York Heart Association (NYHA) standard classification, the heart failure group was divided into three groups (group 1: cardiac function I / II class 2: cardiac function class III). To analyze the correlation between the indexes selected and the grade of cardiac function. All the data were analyzed by SPSS 19.0 software, with P0.05 as the statistical significance. The result is 1: 1. Compared with the control group, the diagnostic accuracy of the ratio of BNP to the disease was as high as 83.5% and the accuracy of CA125 was as high as 73.8.2.The correlation between CA125 and BNP was significant positive correlation between BNP and CA125. The correlation coefficient between them is 0.5660.3. The correlation coefficient between total bilirubin and cardiac function grade was 0.836, and the correlation coefficient between uric acid and cardiac function grade was 0.463nb BNP and cardiac function grade was 0.488GCA 125, the correlation coefficient was 0.6799.The correlation coefficient between total bilirubin and cardiac function grade was 0.836, and the correlation coefficient between uric acid and cardiac function grade was 0.463g BNP. Conclusion 1. BNP and CA 125 have diagnostic value in chronic heart failure. The levels of serum CA 125, uric acid and total bilirubin in peripheral blood of patients with chronic heart failure were increased with NYHA cardiac function grade, which had a good function in evaluating the severity of symptoms in patients with chronic heart failure.
